It comes down to where you see yourself and where you want to go. You've got to ask yourself what do I want to accomplish, how will I do it, what are the benefits. Due to your back issues and how your career aspirations have changed its only natural that your drive has shifted. if your not happy about aspects of it, then you have to take the time to work out where you can fit them in to your day. in a sense, you need a structured day to feel that you have accomplished something. Now I don't mean you become inflexible and rigid, you need goals to reach. I would start by setting a goal and then map out how to accomplish it.
