area of effect or hit boxes in this game can be brutal, which makes me wonder if some no-hit runs are actually done with a mod helping them cheat in order to accomplish such a feat.

If you've played the game enough to understand how far the reach of an AoE attack is/where the hitbox ends, it's very possible to no-hit the game. However, THAT takes multiple dozens upon dozens of hours of in-game study and experimentation, far more than the average player at least.
