So happy to have found this video! Prof. Keating (or Mr. Keating as I knew him back then) was my surgeon when I suffered a comminuted fracture of my elbow on New Year's Eve 2002. He operated on me on New Years Day, and again later in the year to remove the wiring used to fuse the bones together. I remember he called it 'the worst elbow injury he had seen in Edinburgh'. And yet, 21 years later I have no pain and I can still play golf / squash / train in the gym. From my perspective, he is indeed a titan in Orthopaedics. Thank you!

I suffered a shatzker level 6 fracture in 2008. My ortho surgeon said out of the 30 surgeries she performs a month, she only performs 2 or 3 level 6 surgeries a month. She would push them towards the end of the month considering how much time is involved in the procedure. I have 14 deck screws 2 titanium shims and a ceramic hinge linking my knee together. All held in place with human cadaver bone ground down to a consistency of humus. I’m sure a lot of cases are different in the rate of healing however it took me 5 years to re-learn how to walk again. I healed and I’m more than blessed to still have my leg. (I count my blessings ) What I would like to mention is it only takes one contusion to that leg or anyone’s leg who’s had the procedure to throw your gate off and start walking on the hardware in a bad way. Gout in your big toe will throw your entire gate off and start grinding hardware considering your no longer walking on even keel. My Serious Advice is to take the time off your feet for it to heal or you take the chances of jacking up the surgery. (Thank you Dr. Harding!)

About working in Ireland , I worked in same hospital our lady's hospital for sick children in crumbling 1995 and told them never disturb the boss . On my first on call got type 3 suoracondylar fracture I could not disturb my boss Frank Dowding so I call my senior registrar who has just move out of hospital to give me a hand. Also another on call was SHO coving ortho and got a child RTA with C1C2 dislocation with brain dead need consent for organ but my boss ossie Forgarity was drunk and attending a rugby 🏉 match as doctor and was drinking and asked not to disturb him . Was fun

There are many things, evidence based and experienced based, that have led me to admire and respect Singapore and Lee Kuan Yew. Over the years I have come to accept that the Singapore government is well-meaning, rational, capable and benevolent.. However the exile of its passionate political activists such as Swee, Francis, Wah Piow, and the fate of those such as Jeyaratnam lead me to question at times the mistakes that Lee has made. This is a humble reminder that nobody is perfect. Lee's treatment of his political enemies is one blemish that tarnishes an otherwise impeccable and impeachable record.

It was intriguing-I recall reading Miller’s review when I was a fourth-year medical student. At the time, I found myself engrossed in the trauma but was also reviewing the arthroplasty section while in Newfoundland with Dr. Martin. They were discussing releases for tight compartments in TKA (total knee arthroplasty), particularly emphasizing the progression of these releases for exams. Interestingly, Dr. Storey, the chief resident, faced scrutiny from a few staff members regarding this issue. There was a bit of controversy surrounding it, but it was emphasized that understanding the sequence of releases was essential for the examination. Upon arriving at McMaster, Dr. DeBeer mentioned his preference for modifications involving bony cuts. He highlighted concerns echoed in an article or podcast about the uncontrolled nature of soft tissue releases. My initial thoughts dwelled on the unpredictability of soft tissue healing following extensive releases, especially considering the potential vascular alterations in arthroplasty (limitations in applying native joint ligamentous healing evidence to TKA). Dr. Mitch Winemaker supported this notion, acknowledging the limited predictability of soft tissue healing in arthroplasty. I inquired about pie crusting, which he confirmed as a practiced technique, especially in arthroscopy settings. He also shed light on posterior referencing, a concept unfamiliar to me at that time, but one he frequently utilized. Additionally, he directed me to articles discussing pie crusting. While witty, Miller’s Review delved into these releases in arthroplasty settings, a topic examined rigorously. However, I’m unsure of the ABOS stance on these releases, although there seems to have been significant debate, as mentioned by the faculty at McMaster when I joined as a junior resident.

This is important information. I went into the emergency room with a serious tibial plateau fracture and it was misdiagnosed, and i was forced into a splint with an excruciating knee bend. I was stuck like this for 3 days before i could get re-examined and properly diagnosed.
