This was excellent information as I am suffering from this situation and have been for a few weeks. The laxatives only create further discomfort and pain and adding more fiber through Metamucil, doesn't really work. My diet is very good to begin with - what I need is a way to relax my pelvic floor. I would love if you could demonstrate the proper manner to breathe and how often should you do those exercises throughout the day. THANK YOU!

Lie down with a pillow underneath your head Bend your knees, keeping the feet hip width distance apart and keep the feet parallel Take a yoga block, tissue box or book; place it on your low belly Keep sacrum flat on the mat, floor or bed Let all your air out Inhale from in between your hip bones/ pelvis Allow the breath to travel all the way up the abdomen to the costovertebral angle (where the rib cage meets just underneath the breasts); watch the yoga block rise Exhale allowing the breath to travel back down to the hips/pelvis; allowing the entire abdominal cavity to draw back towards the spine; watch the yoga block fall Alternatively you can place the yoga block on the upper chest and as you practice the diaphragmatic breathing, try to keep the yoga block still Take 10 deep breaths 3x/day Pelvic Drop Also known as pelvic floor relaxation As you take a deep breath, imagine the breath traveling all the way down to your pubic bone. As this happens, feel your sit bones widen or spread and you should feel the vaginal and rectal opening gently soften or release down to your feet. It should feel like you are passing gas or gently releasing urine. It is subtle! On the exhale, the pelvic floor will gently lift back in the direction of your belly button. Try 10 breaths focusing on the pelvic floor. Look at the image of breathing and see what happens when you take an inhale--the pelvic floor descends or drops even more into that nice bowl shape. On the exhale it gently flattens up to your belly button.

I went to a new OB today for reoccurring BV and pelvic pain and she was very rude and told me that I need to get it out of my head that my pelvic pain could be caused by BV and that it’s not possible. I left in tears. It made me not want to go to a OB ever again..

One of you said,” what do you do if the patient comes out negative.” Are your tests for BV binary? Or are your tests able to detect potentially problematic bacteria in small amounts? Can you tests detect biofilms that antibiotics may not have removed?

Hi, I found this really interesting. I am from the UK and have erosive lichen planus which has caused me to have a very narrow introitus. I was given plastic dilators by the hospital, and I cannot use them as they cause me a lot of pain. I was told that they only give out plastic dilators as they stretch the muscles better (I think this is mainly for vaginismus as I need to stretch the skin not muscles). It causes me so much pain to try and use hard plastic. I am so much better with anything bendy. How come there are different practices with this (you are using sillicone but in the UK they only give out hard plastic)?

Plastic dilators tend to be on the cheaper side but they are not as effective and painful. The silicone work better because the muscles and skin can soften around the dilator more making the stretch more effective. Hope this helps!

PLEASE READ THIS COULD SAVE YOUR LIFE!!! Bacterial vaginosis is caused by feces entering your vagina that’s why a lot of Doctor says wipe front to back. also it’s not about what you eat it’s not mostly about how clean you are! It’s basically having feces accidentally entering into your vagina causing your pH balance to be off. first thing you need to check if you’re constantly getting bacteria vaginosis is make sure you do not have a hemorrhoid. hemorrhoid are vessels coming from out of your butt which is the extra skin coming out of your butt from straining while you poop, from childbirth & staying too long on the toilet. (Which can leave stains in your underwear leaving feces/bacteria to enter) include fiber into your diet to soften your poop.Make sure you’re not sitting on the toilet too long because that can cause a hemorrhoid. If you are engaged in anal sex make sure before you start you are prepared by using toys and lubricate before you perform anal sex. Second thing you should check to see if you’re gasing/farting a lot throughout the day! That could be signs of H. pylori. Gas coming from your anus often can cause bad bacteria to build up. Stop wearing Thongs & G- Strings! Wear cotton underwear! These doctors will play with you to have you constantly coming to them just so they can say that they treated you and of course they are going to get paid. Have multiple doctors for second opinion. AlWAYS WASH/ RINSE AFTER SEX! WEAR CONDOMS CONTINUE READING AT MY REPLY BELOW 👇 Hemorrhoids it leave feces stains in your undies, even if you wipe well, stains will always be there. as you move around those stains particles can easily enter your vagina. Which is why you’ll always get a fishy order when u get a BV and doctors always ask or tell you, do you wipe front to back. I’ve had BV for almost 2 years, once I took cared of my hemorrhoids and H. pylori(which cause constant gas/farting) , it completely stopped ((I’ve tried everything boric acid, change shower gel to DOVE 🕊 non scented, cranberry pills, probiotics, all the home remedies, including changing my detergent to non- scented, changed my diet, fruits, vegetables, drink half my body weight of water etc)). I would panic because it’ll take me several wipes to clean down there and even when I thought i was clean when i got home I’ll see lil spots of stains in undies, I paid it no mind for a bit because I honestly didn’t know what was going on and why it was happening. Then I started seeing needle like spot of blood when i wiped! and I panic and searched new GYN! Finally I got a good gyn, and told her I’m always sweating down there, always itching, sometimes I’ll have a order and it’ll go away and sometimes it didn’t and I had to constantly wipe 5-10x and I’ll see needle like spot of blood there; did I rip my butt? And she checked it out and told me it looked like hemorrhoids(but i had to show her a picture of my rectum). And I had to go to several doctors for someone to actually tell me I had hemorrhoids which is the sad part. I would always go to several doctors and tell them I keep having BV or severe itching & sweatiness, discharge and slight order and they’ll always say to me nope we don’t know what’s going on, that’s why it’s important to know your private areas well. And would refused to treat me. They’ll give you hints of what’s going on, but you have to try to figure out EVERYTHING your body normally doesn’t do & explain severely to the point you feel like you’re about to commit suicide or it’s interrupting your lively hood to get a good treatment. I’ll also say have a timeline of when you start feeling abnormal.. and take pictures/videos of EVERYTHING THAT IS ABNORMAL OF YOUR BODY TO SHOW THESE DOCTORS. (INCLUDING DISCHARGE) If I didn’t take a picture of my butt she wouldn’t have told me it was hemorrhoids FOR my hemorrhoids- I did the rubber band procedure I went in did one procedure and then I went back in two weeks after the first procedure and did two sets of procedure at once because I ended up having three hemorrhoids. if ure getting the rubber procedure make sure you are off work that day and have nothing to do because you’re gonna need to stay in and not sit on your butt all day. Laying down is the best thing after the procedure. Also make sure you poop before your procedure. It’s slightly painful but nothing crazy you can’t handle long as you stay home and lay down everything will be OK. Also for the 2 weeks you’ll be healing from your procedure, please EAT SOFT FOOD AND TAKE STOOL softeners please… helps you heal and not get another hemorrhoids AGAIN IT IS VERY IMPORTANT TO KNOW YOUR PRIVATE PARTS so if it means taking pictures/video of it and hiding it so when something abnormal happens you can show them, do it! I’m very careful with my health, I eat well, I’m active, I wear condoms etc and I look at my body parts and see if anything is wrong with it, skin, mouth, hair, privates including DISCHARGE . So please teach your kids these things ALSO I know doctor do say men don’t get BV, but tbh I do think they do, have ever had a guy who private smelled? And seem sweaty? If you have, you might have to go to the doctor for them and say that ure having it and treat them yourself, I know it’s crazy but these doctors don’t GAF!!!

Just a question do you know as to why The percentage of African-Americans with BV is substantially higher at 50%, in comparison to the general population? I’m from the UK but I’m sure there must be a similar statistic for black brits here, so I’m curious as to why this is

My pelvic floor physical therapist recommended these same dilators for post traumatic therapy. After 8 weeks of stretching and exercises, she patiently walked me through penetration of the smallest one during an office visit. I have 4 weeks and 7 sizes to go. It is possible to heal from this.

Great question! We hardly ever recommend the glass or plastic. We recommend the glass or plastic if used for rectal work and the EAS is hypertonic. Sometimes the size 1 dilators in silicone are too flimsy and hard to get past the EAS. And yes of course if someone has an allergy to silicone. Hope this helps Andrea!

I've noticed in my experience of pelvic floor pain over a long period that looking at the whole structure is something rarely touched upon. It was a lot of testing the pelvic floor and abs, which ultimately for me resulted in no relief. My issues were resolved by addressing my ribcage which was flaring and pelvis which was tipping forward (a very common postural issue, exacerbated by sitting). A tipping pelvis will result in a tightening pelvic floor and a diaphragm that doesn't function correctly with the pf. Once this was addressed, symptoms resolved almost immediately. Just a thought point to bring the discussion of addressing this as a whole body issue rather than just the pf, in much the same way we should probably look at any pain/dysfunction in the body. Trying to breathe away a problem when not structurally in the correct positions also led to a lot of anxiety! I hope people know these problems can be fixed and to stay positive, I know how lonely, miserable and helpless these issues can feel.

Yes this is very true Andrew! The pelvic floor is certainly just a component to the problem. We 100% agree about working on external dysfunction that can be contributing as well. Abdominal trigger points are certainly an area that must be addressed. It's so great you have learned so much about your body through this process.

I'm 5ft 1, and use the extension table with no problem. My stool goes up and down, so I find it the most comfortable to put my feet flat on the floor and move my stool quite close to the edge of the table- you can totally do it and with each pelvic exam you'll get more comfortable!

So what are the pearls of the toileting education? And because I have celiac I have to clarify that it can only be diagnosed with an upper endoscopy...by a biopsy of the duodenum. Having a colonoscopy myself as the first step cost me another year of anemia and bowel issues. I am fairly new WHNP so I am really looking to be able to help women with urinary complaints. Thank you.

Congrats on becoming a WHNP! We are glad you are thinking about toileting mechanics and bowel/bladder elimination. You can watch our video about constipation and PFD for some quick tips about toileting position. Really, you just need your knees slightly higher than hips and hinging forward slightly (with a neutral spine) will allow the puborectalis muscle to relax! I am sure you have heard of the squatty potty. Glad you got your celiac figured out and because you have been through it, you will be able to sympathize and help your patients tremendously!
