I guess it's a matter of mentality, since it's like a vicious cycle. She didn't move much so her body was unhealthy, then she injured her legs she didn't move at all so her body atrophied, now that she's healed she still retained the habits, mentality and physicality since she was injured so even tho she can move, moving is hard/painful so she didn't want to move. She does need rehab, but maybe there's a mental block that prevents her from doing so somehow?

I can't tell if she's just ignoring how everyone else feels because shes that selfish and awful, or that she is depressed and cares so little about anything, be it herself or others. Dismissing hinano at the end was a red flag. If shes ignoring both her friends and her mother then I really don't know what can be done. As outsiders I'm not sure we can do much either... Bit of a personal story yap no one asked: In my case when I started neglecting my health both mental and physical (Sedentary lifestyle + social isolation), I felt the effects after a few years. Even tho I was 20, despite everyone saying you feel it in your 30s. I isolated myself more and more from everyone around me due to declining mental health, which only exacerbated the problem. Developed problems akin to social anxiety and adhd as a result of it and living on the Internet. Got sick for a few months, feeling nauseous and miserable all the time. Random aches and pains. Realising how this was just the beginning, and it could get so much worse, how much the medical bills will cost for PT, GP, and therapy, and the guilt of putting that onto my parents who would pay for it (and we're lower middle class), not wanting to be as sick as that ever again, my escapes losing their effectiveness (my hobbies started to get boring, this imo was the most important factor bc i couldnt run away anymore. Theres a guy who used video games as an escape and it was effective enough that he died playing games for like 40+ hours straight or something) combined with my mental reaching all time low is what finally made me start changing things. I ended up discovering some sports that I'm interested in. Only started one because I'm still taking really small steps since I know how easy it is to be over ambitious, fall hard and then keep falling instead of taking a small fall, and getting up easily. It still feels hopeless a lot of the time but I know it isn't. I hope she's not beyond that point yet, so she can start turning things around. And I hope she is just pretending not to care. At least she has friends she talks to, which I believe is her saving grace compared to my situation.

I'm glad people in the comments are actually condemning that style of life (if you can call it that way). Not leaving your house is really bad for your health for multiple reasons and very dangerous as well. The less you move, the less your body will want to move, you'll start having less muscle and more injuries at the slightest of movements, and as you grow older, it will only become worse. Also everyone should do health check ups at least once or twice a year. How will she know she isn't sick? Or developing some sort of disease? Cancers aren't detected by staying at home and no one is free from getting one. Also who's cleaning her place is she is so lazy? You can get sick from living in a dirty environment. The last thing i would want is for VSPO management to release some sort of note saying they are sorry to inform Uruha is no longer with us because of this life style. Fans should condemn it if they care for her! Good on Hinano for bringing some sense it this convo. The listless cute girl troupe is only cute in anime.
