Ok: - they start with tainted Isaac due to the swapping between 2 items gimmick (makes finding useful items more likely) - after they find glitched crown (5 item version of the tainted Isaac gimmick) they then obtain lazarus' rags and die to revive as bloody Lazarus, removing the 8 item limit. - they then use this degree of item choice to obtain ipecac (makes your shots explode) and pyromaniac (makes explosions heal you instead of hurt you), which allow him to heal infinitely through explosions - this, when combined with sharp plug (allows you to use active items even when not fully charged, in exchange for taking damage) allows infinite usage of active items - in this case, they infinitely use box of friends, which copies all of your familiars for the current room - the specific familiar they take to copy is succubus, which multiplies your damage by 1.5 when inside its range - this effect stacks, meaning that having multiple succubuses exponentially increases damage - lastly, this is combined with rock bottom which prevents stats from going down, keeping the damage permanently at the absurdly high value it got multiplied to.
