Or use an olsen hegar needle holder which have scissors. I'm a vet do dozens of surgeries a day always "independently" and can't imagine doing surgery without them.

@@imixmuan9081 so true ! But you know when you’re doing a Subcuticular and you accidentally cut your suture …. The heart 💔 😂
@imixmuan90817 күн бұрын
@@MedSyncVT That definitely happens. The other day I was closing a long abdominal incision with a simple continuous and I freaking cut the suture right at the end with my scissors. Option A) take out the whole line and start over. Option B) Do a simple interrupted, leave the tail long and tie the first line to the new one with like six throws. I chose option B.
@MedSyncVT7 күн бұрын
@imixmuan9081 hahahah it's funny because I can see those same options running through my head when it's happened to me too! Using an Olsen Hegar is another way to take the next step in suturing - it's quick for cutting but you know that at any given time if you mis-mount that needle..you're suture is a gonner. Good way to check your focus and precision some may say

Im going to start residency in ortho in couple month from now but when i was on my surgical rotation i saw one surgeon tied knots and the other cut the string and or fixed the knot while the other cuts, cool to see it being managable solo but i hope that wont be nessecary

Wanted to learn this skill for a while!! thank you 🔝🙏🏻
Thank you for the illustration. I just underwent the procedure and my doctor seems to have used the original 4 strand cruciate technique. I have now looked at the few techniques and it seems to me that the midsubstance speedbridge implant system ought to be standardized: it looks truly solid and seem to me that reruptures are practically impossible with that technique. Thank you.
@MedSyncVT2 ай бұрын
It's an excellent repair, 4 stranded a good balance of strength without stopping glide of the tendon. Unfortunately rupture is always a risk no matter the technique used - but the rehabilitation process is equally as important as the surgical procedure. All the best

Why you knot it at the end?
@MedSyncVT4 ай бұрын
For a simple skin biopsy it is not required. However you can diagnose some skin cancers clinically and perform a therapeutic excision in the first instance (BCC and SCC) In cases where you do not close the wound, you place a skin graft or perform a local flap you need to place a marker suture on the specimen. If the specimen comes back as incompletely excised the report will tell you where it is incompletely excised in reference to that marker suture.

@MedSyncVT4 ай бұрын
@MrChristoferoful I mean, you are spot on! Thats the essence of most Surgery.. cut disease out and ideally reconnect it to restore normal form or it comes to you divided and you repair it. The key with flexor tendon repairs is tensile strength . This is 4 stranded tendon repair (the suture crosses the repair site 4 times) which makes it stronger than a 2 stranded or say interrupted stitches around it.
