removed one recently. couple things to beware of. 1. familiarise yourself with the screw retrieval system consisting of picking sleeve and clamping sleeve. if you wind the picking sleeve too far in to the clamping sleeve it'll be v difficult to remove. you will need a small screwdriver to unwind it. 2. i broke 2 crown cutter drill trying to remove the straining screw. make sure you have at least 3.

i am in need of two implants and wanted to know if the ankylos implant system is considered top-notch , strong, long-lasting , reliable, etc, etc. Or should i look at the Straumann or Nobel biocare implants which many consider top of the line? Or is it just marketing that makes Straumann and Nobel that way? If prices are fairly equal between these brands which would you choose? I am considering having this done in either Thailand or Cambodia. Your welcome to elaborate on your professional feelings. I just want them to last the rest of my lifetime without problems or failure. So i am willing to pay a little more for the better system.

This is a complete joke! If an Ankylos solid implant abutment breaks off do not waste your time doing the above - it is an exercise in complete futility...unless of course you want to torture a patient for hours and traumatize them (and traumatize you in the process) psychologically scarring them (and you) for the rest of their (and your) life. Not to mention that your time and the patient's time is worth a lot of money. Far too many things can and will go wrong with the above approach. I have tried it many times - it would take many paragraphs to explain what happens and why - just trust me on this. I have been placing implants for 25 years and placed well over 20,000 of them. I placed Ankylos since the implant was released to the USA market - one of the first to use it. I still place some today. Overall it's a good implant BUT the solid abutment can and does break off occasionally - rarely. If it does REMOVE THE IMPLANT. Do you wonder why they don't show this being done in an actual patient's mouth??? It takes hours to do it this way...HOURS. It generates a ton of heat even with irrigation. It is an absolute cluster "F" to even consider doing this to someone. You definitely would NOT want someone doing this to you. The absolute easiest and very best way to do this is to simply take a 701 bur on high speed and trough around the implant to just over 2/3 the depth of the length of the implant completely around the implant if possible but if not then get at least 3/4 of the way circumferentially around the implant. Then take a small straight elevator and luxate the implant from one side of the implant mesially and then distally - back and forth and the implant will then remove just like a root tip. Then bone graft the site and allow it to heal x 4 months. During that time send the Implant back to Dentsply and get a free warranty replacement. Place that one 4 months later. Let it integrate x 4 months and then place a screw retained abutment and let the general dentist seat a well made crown WITHOUT OCCLUSAL INTERFERENCES AND ASK HIM/ HER TO TAKE THE CROWN VERY SLIGHTLY OUT OF OCCLUSION. This will solve your problem, save you hours of time, and save the patient from one of the most traumatic experiences of their life.

Can I ask how and what point would a solid abutment fracture is it at the screw shaft or on the taper? If so how? Did you consider doing a post and core cast perhaps cutting some internal anti rotation grooves into the internals and cementing it or would there not be sufficient length.

Dear NicholasThis is not a joke. Not at all. This approach is proved and field-tested. The reason why it is shown on the model is the better sight. And it takes only 20 - 30 min. in the patient´s mouth. Better than having bone grafting, two surgeries and morbidity. Nevertheless, I thank you for your recommendation: "let the general dentist seat a well made crown WITHOUT OCCLUSAL INTERFERENCES AND ASK HIM/ HER TO TAKE THE CROWN VERY SLIGHTLY OUT OF OCCLUSION."Just let me add that it is even more important to check the excursion movements. This will prevent fractures.

I just reviewed this video and utilized the (free) provided kit to retrieve a fractured abutment fragment, and subsequently the separated screw threads. It was absolutely simple and easy. It took about 10-12 minutes and I was retaking a fixture level impression immediately after. I had confessed to the patient that I had never retrieved any fragments before, but told him I was confident in the system, and my understanding of it. Absolutely review this system and utilize it. It is (almost?) idiot proof. There was no torture, no traumatization, no scarring, psychological or otherwise. I must assume that Dr. Gadler has never attempted to use the kit since his interpretation was diametrically opposed to mine, and I had zero previous experience. Good luck!
