Whoever told the guy on the starboard stern line to untie it, was responsible for the error. This was the only working line and had they left it in place, the helms person could have used some forward throttle to pull the boat back in line. Then the guy tying the lazy line could have got it nice and tight, in the right spot. As soon as they released the Starboard stern they were doomed. Mucking about with an extra midship lazy to try to pull straight was going to fail without a stern line. They literally rigged the lines to make the yacht pivot on the bow. Which is exactly what it did.
