always listen FIRST - if there is a bruit - DO NOT PALPATE ! You can dislodge a plaque that can R/I a stroke ! auscultate and THEN palpate for a thrill if no bruit is heard AS, PMHNP.

If you think of the carotid pulse as a wave, the upstroke is the first part of the pulse and is followed by the slightly longer downstroke.
