That is exactly what was mentioned in the video if you watch it carefully! If you take any case of good public toilets in India majority of them have been successful because of the additional program that was added to the toilet and handed over to an organisation to manage that space as well as the toilet and have been working successfully until now.

the first thing you need to understand is we are living in india... here there is no public hygiene ... we indians are really filthy disgusting when it comes to cleanliness.. and this is a factual observation.. from gutka to spitting on the roads to throwing wrappers.. to various other malpractices.. we have already made India one of the most polluted countries.. so to expect even a little bit of self responsibility is completely out of the question.. you need to keep all of this in mind when designing public property.. here are some realistic key points. 1) always keep a pay and use toilets. 2) install corrosion resistant hardware. 3) use rust free hinges. 4) use majority stainless steel indian toilets. and 2 western for people with disabilities (as they are much harder to keep clean) 5) use rough non slip stone tiles in terracotta or reddish brown color stone tile or anti slip sintered stone tiles.. because the later is a complete non porous material it will never stain.. 6) use passive techniques wherever possible.. 7) Install stainless steel flush tanks like the one present on trains 8) keep a well trained janitor who is scheduled to round the clock and use commercial techniques and equipment used in malls 9) Install smoke detectors and levy heavy fines on rule breakers 10) make deep sinks using sintered stone using a double wedge sink and install feet washing basin also (you don't want people lifting their feet in the sink) .. 11) use high density polycarbonate for doors or recycled tetra pak doors, as they are light and they don't attract moisture, termites etc. 12) curve all 90 degree sides and corners.. this will improve the maintenance by a huge margin and nothing will accumulate. 13) assign supervisors which inspect these toilets every day.
