It's the worst when you have a dry eye patient, and they: 1: Have excessive tearing, and you have massive mires. 2. Their tear layer evaporates quickly so the fluress wears off and you get extremely thin mires and then the patient becomes agitated and makes the process last longer. Or a patient with glaucoma who use Combigan or the main ingredient Bimatoprost and they have lush eyelashes and they always blink and you're sitting there trying to hold their lid but then they start fixating on a target nowhere near where you're asking them to look... Ah. I love applanation.

Airpuff test is fine. Way better than goldmann applanation. The day my optometrist switched to that was the day i stopped hating him.

Getting rid of accommodation in this measurement, is extremely difficult. I always use test lenses on my eyes for accurate objective measurements of my refraction state.

As always David Guyton is a brilliant ophthalmologist who was of great value as we worked on a proposal to help pilots avoid entry into myopia - if they had the technical and scientific interest to do so.

Of course, you can use your own Test Lenses to accurately measure your own refractive state, to 0.25 diopters. This works accurately in engineering-ecience. Thanks for your review.

Hi, I was not aware of these Retinoscopy problems. I now measure my refractive state, using my Snellen and test lenses. Current status. 20/20, on Snellen. Refractive state = +0.5 Diopter, both eyes. Near? Good at 20 inches.

Dr. Guyton was great to send me a myopia prevention paper by Dr. Stirling Colgate. That was a major discovery about the concept of myopia prevention in my life.

I have two habitual reading distance. 20 inches, ( -0.5 meter) I therefore use a +2 diopter lens. For my Cell Phone, I use a +4 diopter, reading at 10 inches (0.25 meters). Doing this, completely ends the effect of long term close work. This protects my 20/20 vision, so I do not enter into myopia, a second time.

Dear Dr. Guyton, Very interesting - thank you! I had never seen this detail before. I measure a positive state for my normal eyes, by using a Snellen and a plus lens. At 20/20, clear vision. Then I use a plus lens, until I achive "just blur", on the 20/20 line. My refractive state is therefore, +0.5 Diopters, spherical. This assures me that my 20/20 is very solid.

Nice. But, with 20/20, it is doubtful that "cylinder correction", is ever even necessary. To avoid conflict on this issue, I measure my own refractive status, by using my Snellen and Test Lens Kit. Refractive status = +0.5 Diopters.
