Early right sided ECG to see if there is a right ventricle MI should be done. Reciprocal ST depression in aVL supports RCA cause of inferior MI whereas ST elevation in V5-V6 supports dominant LCx occlusion causing an infero-lateral MI. Fill the RCA inferior MI if the RV is affected but not the LCx inferior MI.

Ohk. That makes explains why we only give lactulose and not other laxatives like pur favourite movicol etc. If sugar is the "drug" that we are providing to the pt with hepatic encephalopathy to maintain ammonia levels, can we give some other forms of sugar like ice-creams, fruit juices, sugar drinks etc?

I’ve been on the agency train since 2014 and noticed a LOT of change in agency’s and lack of care,support and integrity within them. You MUST advocate for yourself because your agency won’t. I’ve seen many a good nurse be discarded by both agency and the contracting hospital. Remember the hospital is the client of the agency, you’re labor for-hire, basically a sole trader. I could tell you a lot of horror stories both in and outside the hospital

I developed edema after a burn once, I was going to buy a syringe and draw out the fluid, but I decided to wait a few days, and the fluid was easy to squeeze out by hand. It came out as a redish liquid. It's obviously a side effect from swelling.

So I walk in on my hunched over shallow breathing patient and I see their sat at 80%... I sit them up, tell them to take some deep breaths and watch their 02 climb. How I think about this situation clinically is: The positional V/Q mismatch is now fixed because air can ventilate more of the lung tissue and the deep breaths are recruiting more alveoli increasing the surface area available for perfusion to take place. Am I wrong? Does none of that matter because Im not increasing the concentration of the 02?

ok last question i promise. How does one determine how much FiO2 will be needed for the patient? would it be buy obtaining an ABG and seeing what the PaO2 is?

The default is 0.21 but if the patient demonstrates desaturation =spo2 <94% it’s reasonable to apply oxygen in small increments to achieve spo2 between 94-96%. Or a paO2 >80mmhg

Im sorry, another question, so would the RQ also vary depending on the diet? Or is the 0.8 a standard number used for all patients in the equation (assuming they all have a balanced diet)

Totally yes. So RQ is based on a balance of cho, lipids and proteins. More carbs and RO increases, more lipids and RQ drops. The standard equation assumes a balance diet.

Yes absolutely, so I get my students to pretend it on the top of Mount Everest and then calculate what the PA02 Would be for a climber at the Summit. To do this I need to find out what the barometric pressure is on the top of Mount Everest (about 230-ish) then plug that into the equation.
