Man, after 70 episodes and everything he's seen and been through, Tenma is still as capable of keeping his composure as a 2 year old toddler. A brain surgeon being a wimp is the most unrealistic part of this show. Every surgeon I know could get their guts pumped full of lead from an AK and would just continue looking at you deadpan without a change in their expression. Tenma loses his composure at the smallest possible inconvenience.

An interesting interpretation I found online: Johan's empty hospital bed could mean a number of things. Of course, the open window nearby seems to suggest that Johan fled and escaped. But the earlier scene where Tenma confronts Johan complicates things. There is a moment where Johan suddenly rises from his slumber to "speak" to Tenma about his mother. But when Johan finishes, Tenma blinks and Johan is once again sleeping. This heavily implies that what Tenma saw was no more than a hallucination. But this begs the question: if Tenma hallucinated Johan speaking to him, who's to say he didn't hallucinate Johan's existence? Perhaps Tenma never saved Johan at all, and he is simply speaking to Johan's corpse, or even an empty bed. And if Tenma didn't save Johan, was it out of his control, or did he allow him to die on purpose? Another interesting interpretation is the one surrounding the nameless monster: His mother had given Johann and Anna names, but Bonaparta took them away from them and Johan only got his from General Wolf, from the book "The Nameless Monster". To this day, no one knows to what extent coma patients can perceive their surroundings: Tenma told him what name his mother had planned, so perhaps the Nameless Monster in Johan has now finally rested!

Interesting theory but if Tenma hallucinates Johan, it's not like he hallucinated his whole existence. My interpretation is that when Tenma visited Johan in his room, it was already empty, and the whole sequence was just inside Tenma's head. As for you 2nd interpretation, it's not bad but just there's nothing indicating that Tenma told his real name, only that he had one.

Most of the people Johan Manipulated in the show are rather dumb and quick to fear to any violence while Ayanokoji manipulates more smarter opponents sure sure Johan can make people commit suicide by using nihilism or using edgy words but having greater influence or mass scale manipulation or making 52 kids die doesn't make you smarter koji defeated a supercomputer as an kid and survived much tougher conditions we do not know johan's psychical capabilities expect for statements such as he is a perfect soldier or such things johan can't manipulate Ayanokoji as he doesn't fear anything in an fixed situation koji destroys him in real life situation johan can send a killer to shoot koji and his friends or cause chaos in the school

I think the "monster" in question is a person belief that you don't matter - in that moment when his mother chose him, but then changed his mind, Johan begun to believe that he has no value. He is not a human, he is a monster, and thus, he doesn't fit into human world - in fact, human world has no need of him, which is why he aims to do his "perfect suicide". Johan always messed me up because, like, fuck me man, even if he got better, I don't think he would ever get 100% over his trauma.
@yamatonadeshiko567

Hhmm The only outcome I could think of was Johan actually died. Guy wasn't intubated or had no mechanical ventilator attached to him. He had no nasogastric tube inserted for him to be fed. He was in a coma for a month or more. A big chance he also had muscle atrophy or edema due to long periods of inactivity or lack of nutrients thus giving him a hard time walking or running. He will have to relearn walking through therapy.

Because this is the scene where we as audience can also see the monster clearly which Johan was trying to show us... It was the belief that every life is not equal, yes! this belief is the real monster which exist in many humans just like he saw it in his mother but when Tenma saved him, he understood that this monster is not the ultimate reality and a human can become rea human who can love everyone equally. So yes Johan finally became a human again in the end. And in this scene it was not real Johan, it was just Tenma's hallucination which helped him to understand Monster that Johan was talking about.

Personally I think that the twins mother knew which twin she sacrificing. She was about to give up Johan but quickly changed her mind! The one thing that really impressed me in this anime was how much i was terrified of Johan whenever i saw his shadow, face or even his feet. There were about 20 episodes where you heard everyone talking about him but we saw him not a single time. And then as we met him again after such a long time i nearly pissed my pants even though ive seen scenes and pictures of him before and i just thought hes handsome.

I really loved how the concept of who lives and who dies was done in this story. Tenma as a genius brain surgeon essentially is a god who decides peoples fates. Similarly to how Johan’s fate was decided by his mother all those years ago. When Tenma saved Johan the first time, he became intrigued with the idea of having someones life at the palm of your hands. In his own sick way Johan decided he was going to follow in his gods (Tenma and his mother) footsteps and be the one who chooses who lives and who dies. Including himself. The only difference being Tenma believes all lives are equal in birth. Johan believes they’re only equal in death.
