The insulin to carb ratio is a bit more complicated than what you said. Fats and proteins will affect insulin absorption and then there is the glycemic index as well as physical activity. I don't know how it can be done.

When the image is morphing and going in and out, is that you scrolling up/down or side-to-side in the body? I'm trying to orient myself while looking at these images and preparing for NBME CBSE

On axial CT images which is what he’s doing for most of the video, scrolling will be up and down in the body. Imagine the pt is lying on a bed and you are looking up through their body from the view of looking up through the feet. So in the thorax scrolling up will move towards lung apex and you’ll start to see trachea, thyroid, etc higher up. And as he scrolls down you’ll move into mediastinum seeing great vessels, heart, etc until he scrolls all the way down which will cut through diaphragm and enter abdominal cavity
