Best thing to do is ask the patient, rather than the Caldicott Guardian or anyone else. Just because you might want your data shared, doesn't mean the patient wants theirs shared. Its called consent.

I have fought MS for over 50 years to have had my MRI notes destroyed in Leicester General hospital.A GP in 2009 told me that I can die any day in .I worked on many ward in our NHS saving lifes when I handed in my notice it wasn't accepted....Now I need help no one will help me.

Wonderful to hear and see Hugh and Tara. I am forever banging on to anyone who cares to listen at Alphalake AI about IFTTT digital workflows and UX happening not in a single place and time, but over a user timeline, be that thinking in terms of that users day or week and WHEN/HOW they need to have information surfaced to them and how to encourage in-the-moment processing. This can in fact be a series of messages, triggers or interactions throughout automated and integrated rules-based workflows. The recognition that it's the backbone of interoperabilty combined with these pathway user experiences (workflows) and the need for user designing/thinking/implementation with this in mind is really great to see and hear. I love the harvesting thinking Hugh! Innovation needs to operationalised and then in turn operational experience and outcomes fed back to drive more innovation. I'd add here that it's also about having a more iterative mindset. Vast amounts of money is lost in healthcare in big bang / waterfall software implementations that totally fail to realise excellent UX ONLY happens through excellent software versioning, which in turn needs excellent user voice feedback into the product. Whilst we have an every 5 years "Upgrade" or even every year upgrade culture, we'll continue to struggle to keep our digital ecosystem up to scratch. Even 2 version releases per year would be, as I see it, slower than ideal, but better than how things are dealt with in a lot of areas currently. Iterative mindset is key! The process of enabling great UX needs to be better engrained. Conversations and initiatives like this from NHSX are a huge positive step towards this.

@@louisemarsh6106 Hi, not to be rude, just wanted to mention another old saying: "measure twice, cut once." I agree a real doctor is necessary, but it also never hurts to have a second set of eyes, especially if those eyes have seen hundreds of thousands, if not millions, of situations/scans. It's by no means a replacement, but rather another tool in the box to help doctors improve diagnosis and catch things early.

The only problem with digital care that patients have less access seeing clinical staff face to face including problems accessing medical records to provide medical evidence to other Government organisations like DWP PIP and UC medical accessments. Future digital will require patients giving full access to medical records to be transfered to DWP for medical accessments knowing during the Covid-19 problem thousands of doctor's have failed to write medical reports for peoples assessments.

The biggest problem in care homes is the pandemic of ABUSE in them. Every single care home. If you have a family member in a care home. Get them out. The staff are only nice when you visit.. they leave them on commodes next to their beds for so long they call the police. They sit asking for the toilet for two hours ignored by staff. They have bed sores. They are not listened too when they are in pain because they have dementia. Non of the staff want to to be there. The gardens you see are like north Korea... The residents don't get to see them Because they have to stay inside hotel California all day. The gardens are for show.

Shouldn't this video have addressed system integration and semantic interoperability between electronic medical records in the health and social care domains facilitating a fully integrated summary care record for each patient and supporting the "long-term plan" for integrated care? This video is almost meaninless.
