You forgot unplanned overload and self destruct you get UO by restableiseing the reactor over and over again and self destruct is caused by shutdown (the chance increases each shutdown you can see the chance on the laptop in the server room

Actually, Unplanned Overload is when pre-meltdown occurs with different music and something bad about stability, and when you restabilize it, unplanned overload happens.
