Hi, On the Dicom tag : (0018, 115E ) it stores the DAP value eg 1.149 with no units. It is known that DAP is expressed in Gy.cm2 but under dicom standards its given as dGy.cm2. I know they just differ by prefix and contextually they the same. How would I know what units my DAP value actually is ? My X ray was captured with a Siemens machine, and the image was complemented with a Dicom info. Since it follows Dicom standards can I assume it'll use dGy.cm2 as its units or could there be an exception to this ? Basically I'm asking if a system is following dicom protocols then they must have the the same units stipulated by Dicom standards or can they differ i.e instead of dGy.cm2, they present the value stored in this tag in Gy.cm2 ? Wow, great question. Because it is not a required tag it is technically possible that a system could be configured to store the DAP value by another measure, say mSv. However to do that would require some serious DICOM work and probably involve configuring the modality itself to convert the DAP value into the other value. I can't think of a practical reason anyone would want to take that challenge on. In short, is it possible? yes. Is it likely someone would actually do that? No way.

Hi Mask, Thank you for the kind words. I'm alive and well. I use KZbin as a storage space for the videos I use in the courses I teach at college. I never imagined anyone other than my students would want to watch them but I'm thrilled many people find them helpful. Haven't needed to make any new videos lately however, there will be new ones posted probably in August. I'm adding a chapter on FHIR to the HL7 course so there will be videos for that. How does the MWL SCP know which job is done? Does modality sent the information back to MWL SCP?

Hi @yllantw, there are a couple of ways that can be done. If the modality is using modality performed procedure step (MPPS) the modality can track when the exam was begun and ended. The other way is to have the techs enter in the RIS system when the exam was begun and ended. The important thing to remember is the MWL SCP is looking for exams that are at the scheduled step, once the exam moves passed scheduled it won't be found by the MWL SCP.

Why is it that annotations don’t cross over from one PACS system to another? Ex: from Philips to FUJI. The annotations on images from Philips don’t come across to FUJI when it’s received.

Hi Dynecs, that is a great question! Without the log files and each systems DICOM conformance statement I can't give a definitive answer. However, I can offer a possible explanation. I suspect that the raw image is being transferred from the Philips PACS system to the Fuji PACS system which would not include the presentation state DICOM objects. In simple terms the image is created and sent to the Philips PACS system. Then the annotations are added and stored almost like a text overlay, creating a second version of the image. When the image is transferred from the Philips system to the Fuji system the original image is sent which doesn't contain the added annotations in the overlay.

I’ll have to do some digging in the logs and figure out what is being transferred and what’s happening with the images in the DB post annotations. It could be that two versions are being stored. I look forward to more questions! :) Another thing to consider is that even if the Philips PACS is storing the annotations as a DICOM object (there are a couple ways it might do that) the Fuji Synapse system might not utilize the same DICOM object. There is a good chance to make the annotations cross over you will need some serious assistance from both vendors.

Que puedo hacer ??, ya fui al lugar donde me tomaron la tomografía pero ahí me dicen que de esa forma copian al dvd, no tienen otra forma.. Hola Noelia, Eso no es inusual. Muchas veces, las imágenes tomadas en un lugar no se pueden cargar en otro lugar. Aquí hay un par de cosas que puedes probar. 1. Obtenga un DVD en blanco y haga una copia del DVD original. Entregue la copia al otro hospital y podrán ver las imágenes en el DVD. 2. Si observa las herramientas u opciones en el DVD, es posible que pueda exportar las imágenes en un formato diferente. Si puede hacer eso, puede hacer un nuevo DVD o simplemente guardarlo en una unidad flash. 3. Si se trata de unas pocas imágenes, como un examen de rayos X simple, siempre puede tomar capturas de pantalla de las imágenes, guardarlas como jpegs y enviarlas al otro hospital en ese formato. ¡Espero que ayude!

great question Unicorn. I do not know of any cases where someone would convert DICOM to HL7, that would be extremely difficult. However, a modality worklist does not have to be DICOM based. Many places have HL7 based modality worklists. In an HL7 modality worklist instead of querying the PACS system for the worklist the modality queries the Radiology Information System (RIS) for the data. Hope that helps!

In this context a modality is a medical imaging machine. A ct scanner is a modality. An ultrasound cart is a modality.

Hi Reach, the list is just a list of patients/exams waiting to be done. However it's not really sent to the modality, it's a query-retrieve function. The modality queries a worklist server run by the RIS or the PACS and retrieves the list of patients/exams waiting to be done that are scheduled for that modality. Hope that helps.

Great introduction to DICOM! One observation: At 3:50 did you mean CD readers, not burners? If a patient brings in a CD to be imported, it will be put in a CD reader, not a burner. Right? Or am I confused here? Thanks again for your helpful video!

Hi Endicott, great question! By CD burners I meant PacsCube or Rimage machines. These are machines used primarily by file room staff to burn CD's for patients. However those machines are also capable of reading CD's that patients bring in from other hospitals and then importing those images into the hospital PACS system. Thank you for the question!
