This guy is so relatable he symbolically humbles himself by sitting on the ground. In so doing he demonstrates his theoretical mastery by evoking connotations of 3rd wave behavioralism and mindful cognitive approaches, as he simultaneously, and literally, leans on the symbol of psychodynamics: the couch. He has metaphorically gone beyond what many consider to be the foundations of the field, transcending to a new postmodern integrative feminist-informed level where he can meet the client in radically mutual collaboration.

Thank you for sharing this helpful piece of content. Just a quick question. Can a counselor share everything, including therapy notes, with a third party after a client has signed consent? Or does the counselor have the right to withhold some information?

Hi there! This is a great question, and the answer is a bit tricky as it will change based on the context of why the information is being released. As a general statement, even when we have the client's permission to share information, sharing less is best seems to be the approach many professionals take. For example, we might have a client who we are assessing for an autism diagnosis. As part of this, it's not unusual to talk to the teacher to gather information about what's happening at school from the teacher's perspective. Once the client has given permission for us to talk to the teacher, and when we talk to them, we would not have to say anything to the teacher about how we are assessing for autism or any other information related to treatment. Instead, we could simply ask the teacher to share what they've noticed. That said, it's not impossible to imagine times where we might need to share more information, such as notes. An example of this might be if we have had to assess the client as part of a court ruling. It's important to keep in mind that the information (and confidentiality) belongs to the client, and that they can make choices about what information we do or don't provide. At the same time, we should be prepared to help the client make informed decisions about their information. I feel this question also highlights the importance of good informed consent at the beginning of treatment. I hope this helps!

Is stopping a client from engaging in self harm a violation of the first principle - autonomy?

This is a great question! If we look only at autonomy, one could make an argument that stopping a client from engaging in self harm would be violating the principle of autonomy. That said, there are other ethical codes that we must also balance. Meaning, we have to consider the self harm in the context of all of the other ethical principles as well. Not only that, but how would one truly go about stopping someone from engaging in self harm, and would that in any way violate ethical principles. Also, depending on one's idea of how they provide care, it might be helpful to work with the client with a harm reduction approach-for example, encouraging them to hold an ice cube rather than cutting. However, some facilities or therapists might use a more abstinence approach (meaning they would require the client to stop drinking or not allow self harm to provide care). Additionally, it would be important to consider state laws or other policies at there facility where one is practicing. Again, great question! The answer (as you can probably see) isn't super straightforward, and would likely depend a lot on the context of the self harm as well as other factors. It's also important that I mention that other therapists might also bring other factors into this conversation, and that these are simply a few of my initial thoughts from your question.
