Good question. Herbal medicine can provide support. And the rightful question is , How much can you do at home and how much professional care do you need. Every therapy has strong points. There are some who can treat periodontitis without my help. So let’s talk about what would happen if you were a patient in my office. I would look at you and tell you what I think is indicated. I would give you basic oral hygiene instruction, and do the work that’s necessary, in my opinion, to get resolution of the case. But let’s assume that you say to me, “I want to try herbal medicine first.“ I would then ask you, “how long do you need?” You’d then tell me. Unless it’s an emergency with abscesses that are well entrenched under the gum line, I don’t have any objections to doing everything you can. I would re-examine you three months later and see how well you were doing. We then be able to compare results using your individual therapy without mine. If there is no calculus on the surface, your therapy maybe all this necessary. I don’t know. if there is calculus on the surface, calculus is toxic. And while things may look better, they may not quite be good enough. At that point, we could agree that the calculus should be removed and that would be the next step. I participate in care. And if they are good in herbal medicine, homeopathy, Ayurvedic medicine, I am perfectly good with that. Here, however, is where patients have gotten into trouble treating their own disease. When oil pulling was popular, about 10 years ago, I had three patients who allowed periodontal abscesses to progress, trying to treat those abscesses with oil pulling. It didn’t work. And a great deal of bone was destroyed as a result of their treating themselves. Recently, I had a person in her late teens who had been treating herself with remedies that her mother recommended. And the patient was getting worse as result. Therefore, in my opinion, you need a professional observing the progress. Professional intervention is sometimes indicated. So find a dentist that you can trust who is willing to work with you. And that way you have the advantage of professional monitoring while you do the herbal treatment and you’ve been trained to use. Periodontitis is a chronic degenerative disease. So just as any chronic degenerative disease, inflammation is key. Processed foods and sugar are to be avoided. Fresh fruits and vegetables are to be emphasized. An excellent guide is provided in the recently published book, Good Energy, by Casey Means, M.D.

This is a really important message. I should probably show this to patients because I have heard many of them say that they don't want to hassle with their teeth anymore, so they'll just get them out and get implants, which they imagine will solve all of their problems. Of course implants don't get decay, but they are definitely not the end of all issues. Also, my success rate at convincing people to stop eating sugar probably hovers around zero percent, although I hope a few people have made little changes, like quitting sugary cough drops.

It is great to hear from an endodontist. We do a lot of implants in our practice. Implants however are subject to peri implantitis. They are more prone to that when they’ve lost their teeth due to periodontitis. It’s much easier and more predictable to treat a periodontally involved tooth then to treat periimplantitis. That’s one more reason to treat the tooth rather than extract it in favor of an implant. As long as it is only minimally mobile, we can nearly always treat it.

i have pretty rampant decay and teeth in the back that are heavily decayed like to the gumline but no abscesses i can eat just fine still though some foods I avoid now like really hard foods they want to pull everything but just seems really scary when i'm so young I don't want to look old or ugly I'm not sure if i'm a candidate or implants or not i have GERD and MCAS how do know if FME is the right choice?

Matt, you have a difficult problem. As I mentioned, rampant decay is difficult to treat. Let me give you some advice: 1. There is a KZbin site with Dr. Ellie Phillips who provides preventive techniques you can use. 2. You will need to completely stay away from sugar. 3. It’s important to get to the source of the acidity. Working with a physician, perhaps a nutritionist, can help you identify the source. You can buy pH paper to check the acidity of your mouth. Whatever you can do to control the acid and maintain a pH of 7.0 will be essential for preserving your teeth. 4. Find a dentist who is willing to work with you despite the rampant decay. Sometimes, fillings can be done to help keep things going while you work on getting things under control. Large fillings can be done, but if the decay involves the nerve of the tooth, a root canal may be necessary. However, it may not be worth it if the source of the decay remains, because, as I said in the video, if your mouth is full of acid, no matter how good the dental work is, the teeth are likely to continue decaying. I hope this gives you some helpful hints. It’s important to get a second opinion to explore another person’s approach to your problem. I wish you good luck. Lee Sheldon

I understand your concerns, but let’s take a different perspective. Rather than dismissing root canal treatment entirely, it’s important to recognize its value when performed under the right conditions. I regularly refer patients to endodontists for root canals, and when handled by a skilled endodontist, the chances of resolving the infection and saving the tooth are quite high. Of course, no procedure offers a 100% success rate, but if the root canal is done well, the tooth isn’t fractured, and it can be restored properly, it gives patients a solid chance at keeping their natural tooth. While we perform many dental implants in our practice and are confident in their effectiveness, preserving a tooth, when feasible, is often the best route. If the right factors are in place, and the endodontist has the skills and tools to perform the procedure effectively, saving the tooth becomes a very real and beneficial option.

You know you really need a root canal when you consider it the best thing any dentist has ever done for you. I am that person. I felt fantastic the next day, like a veil of head fog had been lifted. I felt alive again. I still have that tooth, 15 years later.

Visualization is necessary to not only provides the awarness of whats below the gumline, but confirms we get the root surface clean. It's possible to be better at what we do for our patients. As a clinician with over 2 decades specializing in dental endoscopy I know. Dr. Sheldon is spot on. My first goal is to treat infections, then observe. The second goal is restoration. Nearly all periodontitis is treatable. It’s a matter of diagnosis and appropriate treatment. Rampant caries is far less predictable. In future videos, we will take increasingly harder cases and I will spell out the sequence that we take and give to the patient.

If you place temp crown more coronal(”at correct gingival margin”) so that gingiva migrate coronally as well, how do you manage marginal integrity. Aren’t you gonna have open margins?

You will create a new margin in the more coronal position. The axial preparation will be narrower to create the new margin.
