The reason is why it's dying is because you're not cleaning the tank. It looks really mossy and gross,and it might have some amoeba in there.

WHY IS IT EVEN BEING KEPT IN THAT TANK. YOU HAVE TO TAKE CARE OF THEM. you have to Clean the tank Change the water Add a filter (you don’t have to but it’s better) It has to have hiding places, or it will be stressed AND YOU CANT LET LITERAL *ALGAE* FORM IN THE TANK
