Great to see! What about disc bulges? I caused quite a few working out improperly (slight twisting and leaning with heavy weights and complex movements). I feel them still. They're about 2mm in the thoracic and cervical (a few annular tears too). Do these ever retract or become smaller? I have looked for evidence of it, but couldn't come up with any MRI examples. I know that symptoms can decrease, but mine are still bad after a year. I have mild endplate irregularity too and that may be from the awkward workouts too (although, Modic changes weren't noted directly). I'd love to see any proof of disc bulges decreasing naturally.

Thanks for the video....Just subscribed........What if I have all the horrible symptoms of nerve compression (neck pain, numb hand, weak limbs, bicep pain, heart palpitations, hot flashes or red face etc) My mri and xrays show bone spurs; arthritis; and minimal disc bulg at c5-c6 and c6-c7; BUT the experts looking at them say that the pics don't explain my symptoms.....terrifying situation because symptoms getting worse over time.

I'm curious about your thoughts (or from others here) about endoscopic approaches for decompression that don't involve fusion. I've seen a neurosurgeon who suggested a 4 level ACDF. I'm very concerned about that because of the possible need for a revision down the road due to adjacent segment issues. If decompression can be achieved without fusion, I'd be really happy. I'm just not certain about the efficacy of that procedure. I realize that ACDF has been around for a long time. It just seems like an endoscopic approach is much less invasive, lower blood loss, and shorter recovery time. Thanks for any input from anyone here.

Both work very well, it really depends on symptoms and imaging that a experienced cervical surgeon can explain carefully to help you achieve your goals

I have cervical stenosis had injury . And symptoms gotten worse over year , it started in left arm now both arms , I can not use both arms, doctors keep going back n forth.. say it could be in my head. But I'm losing muscle n function of my arms.

Thank you for this wonderful presentation. I just wanted to point the fact that the patients have no control over the waiting times before they get their surgery especially in Canada where I live. It takes at least a year to have an MRI done followed by at least another year to see a spine or neurosurgeon and more waiting before the surgery finally happens. It is imperative to shed more awareness about this condition, especially as the prognosis after surgery is much better if it’s performed within 6 to 12 months from the onset of the noticeable deterioration. Thank you again for being passionate about this topic

i am a 51 years of age. have both myelopathy and radiculopathy. started with severe back shoulder pain and electric pain shooting down my arm at the worst when running jump roping during my cross fit work outs. ortho surgeon order MRI- (had one from over 1 year that only showed DDD at c5 and c6) now tolf have budge disk and stenosis that is flattening my cervical spine at those disk area. Doctor , said i have some weakness on left side of arm and hyper reflex on knee cap test. HE told to cease cross fit and any impact sport as it is damaging the cells inside my spinal cord. This is a huge set back for me to have to stop my fitness work outs when dealing with pre menopause symptoms. I had an epidural injection that relieved the radiculopathy symptoms for the first 2 weeks. The Surgeon is recommending a 2 level subtotal corpectomy. He explained that he would use an ultra sonic tool to remove the stenosis around my nerve and fuse my c5 and c6 in. he said due to my age this will help me been able to work out after 3 months recovery although is recommend I switch to something else than cross fit. I am getting a second opinion from a neuro surgeon only because I was not offer any other treatment options. I just downloaded the Movemed app. What do think about my case?

Thank for this very thorough presentation, it’s very informative. I’m a 60 years old female, retired paediatrician since 2 years. I experienced some subtle pain and weakness of my right hand and wrist along with loss of temperature sensation in 2013, had a normal EMG normal head MRI but no neck MRI was ordered then, my symptoms lasted for few weeks and resolved spontaneously. After that I started experiencing intermittent neck pain with limited range of motion along with episodes of unbalance, lower limb debilitating cramps, upper limb and skull allodynia, this led to a sole diagnosis of Fibromyalgia that lasted for about 5 years and wasn’t questioned by my providers despite me having most of my symptoms on the right side and developing increased lower limb weakness along with progressive loss of mobility and extreme fatigue. I ended up retiring at age 58 to be able to recover as I was made to believe that all my symptoms were related to anxiety and stress. Retiring helped with my fatigue and reduced the frequency of my symptoms, however as I became more active doing some walking and hiking in Spring 2023 I developed shouting electric pain in my arms along with scalding sensation of my upper body that finally prompted my doctor to order neck X-rays and cervical MRI which showed severe protruding disc at c6c7 along with multilevel foraminal stenosis and degenerative disc disease. These findings were made in September 2023, in the meantime I haven’t been very active except for some indoor biking as it’s the only activity that I tolerate without triggering any pain. Walking causes me severe mid back pain along with numbness of the right thigh and sharp heel pain, I had a lumbar MRI that showed multilevel mild degenerative disease with mild foraminal stenosis. My painful symptoms still come and go but my loss of mobility is progressive, I had an EMG that was normal, and I had 2 neurological examinations one performed by a physiatrist that was positive for both upper and lower extremity hyperreflexia along with a positive Hoffmann on the right hand, the other exam was performed few weeks later by a neurologist and was negative. Based on the EMG my providers put a hold on consulting a spine surgeon and preferred to keep relating my symptoms to Fibromyalgia which by the way is not a convincing diagnosis for me as most of my symptoms are on the right side and there are major positives findings on the cervical MRI as I mentioned before. I feel strongly that I should have a spine surgeon opinion and your presentation convinced that I should fight to get it as soon as possible. Thank you so much for this informative video.

I was told I had myelopathy and had 3 level ACDF 2 years ago. My arm became numb and my hands weakened. Also progressing back pain along scapula and shoulders. Original surgeon said partial fusion at C6/7 and put in Dtrax wedges. No improvement. Xrays and CT show foramen compression, severe. Saw a neurosurgeon and had myelogram. That report says Partial bony fusion across the disc space is seen at the C6/7 level with incomplete fusion laterally. There is no bony fusion at the C4/5 level. Original surgeon says I fused completely. Not sure what to do next, if anything. Thanks. And thank you for this interesting video.

Hopefully the a&e in RVH Belfast get an update...I was sent home 3 times...last time by a neurologist reg who lied told me I'd see neurologist and have mri in 2 weeks! I'd been suffering from Oct 19 and had to have emergency posterior laminectomy after eventually being admitted for 6 weeks and still sent home with wrong diagnosis end Feb 2020..op April 2020 and I'm left with permanent damage and complete life change! So cross I knew there was something wrong with me but just ignored!

Hey doctor I have a question I had a mri on my neck like about 9 months ago. Was told I have Cervical Myelopathy with significant cord compression c5 c6. I have symptoms of 24/7 constant lightheadedness/dizziness that never goes away. When I bend over to pick something up from floor, it intensifies my lightheadedness. Been a couple years now feeling these symptoms. Had vertigo twice also (where It feels as if room is spinning). Other than that im brain fogged with lightheadedness/ Dizziness everyday. Does it cause any of these symptoms im explaining?

Hoffman sign is an involuntary flexion movement of the thumb and or index finger when the examiner flicks the fingernail of the middle finger down. The reflexive pathway causes the thumb to flex and adduct quickly. A positive Hoffman sign indicates an upper motor neuron lesion and corticospinal pathway dysfunction likely due to cervical cord compression. However, up to 3% of the population has been found to have a positive Hoffman without cord compression or upper motor neuron disease.

I've been self-treating a cervical herniated disc with: 1. spinal decompression blow-up pillow 2. red light therapy tried many other things, but this combination along with inflammatory herbal ointments has been the most effective

What are your opinions on pro disc cervical? And is it possible to still have cervical adr after endoscopic posterior lamintomy form at c34 with 1/3 facet drilled. I'm getting mixed reviews in nyc.

The pro disc is an excellent implant with years of international data supporting it’s use, I use the lumbar pro disc with good results A disc replacement can be carried out after a small laminectomy but very much depends on the amount of bone removed and facet involvement. Results can be variable. An ACDF is probably a safer option in this situation but get guidance from a number of surgeons

Infection rates with all disc replacement systems are traditionally very low. I am not aware of any conclusive data showing higher infections with one implant over another. I don’t use the M6 implant but it has excellent long term data supporting it’s use. Osteolysis and heterotopic bone formation are more common risks with all cervical implants. Other risks include but are rare cerebrospinal fluid leak, nerve root injury, esophageal injury, dysphagia, prevertebral hematoma, prosthesis migration, implant subsidence, postoperative C5 palsy, and hoarseness
