This was the first time I have ever heard anyone make that comparison in an interview. Thanks for sharing Dr Quinlan, In addition to the tools of consistency, self care and other important tools mentioned, the use of soft skill of communication is an excellent tool for interacting with patients of diverse attitudes and attributes. Studies of current research show that when clinicians/healthcare providers are compassionate and strive to make more genuine connections with patients, their roles are more fulfilling, thus reducing the risks of managerial and provider burn outs.

Excellent points. True leaders take the time and patience to listen to and implement new ideas, to take chances and step out of comfort zones and in doing so, they lay the foundation for creating better leaders for the department. OK, I have no problem with BSN education EXCEPT that BSN graduates do not have the same level of hospital patient nursing care that their counterparts in the old hospital schools had. I was fortunate, after not making it through a hospital school, went onto get my BA and then was a Certified Tumor Registrar for almost 10 years. I went from 250-650 patients/year and counting and ran the Cancer Management Program at my hospital, and while I had the support of the MD's that I worked with, I did not have the support of management, particularly my boss who was head of medical records. My dad offered to pay for everything, including tuition, so that I could concentrate on nursing. I clepped A&P and Microbiology after 10 years and passed them both without studying. I had to take a chem 101 course as well to get into the Quincy College ADH program, which I passed. I missed cum laude by less than 0.5 of a point. This was after I was invited to a dinner specifically for the President's Award for Academic Excellence. After a brief tenure in nursing home as the night charge nurse for both Medicare and long-term units, I decided that my skills would be better served by doing agency. I then went into home health and while I've done a bunch of other stuff along the way, home health is where I can make the most difference for the people that I have served. I am not in favor of the hospitals buying up all the VNA's and making "networks" and that "the nurse will follow along the continum." Home health is as different as putting me into one of the units or ICU. They are entirely different kettles of fish. Furthermore, in their quest to keep people out of the hospital, Medicare with deals from AARP and AMA and Big Pharma has screwed the patient population that I will find myself in next month--65 and over. Observation beds have gone from 3 days to 5 days and the patient gets stuck with a huge bill. People who are on Medicare or Medicaide (unless they have a really good Medicare plan (and can afford it)), are treated differently than those who have regular insurance. Sad, but true. In addition, VNA's are coming up with protocol for CHF'ers which, in my humble opinion are terrifying. You don't leave a short-line IV in for more than 72 hrs. That particular protocol wanted it to be 5 days. You don't leave a patient to whom you have just given IV Lasix or Metallazone, go and see another patient or two and then come back. These people can turn on a dime, just like babies can. I am not in favor of this constant downsizing and a whole company taking over a whole state like VT with a single one-size-fits-all site where you can apply for a job. Whatever happened to free enterprise? It seems that instead of practicing good medical care by the vision expounded by Dr. Tim Porter O-Grady, there are already a lot of unhappy people being served by agencies that cannot get good nurses because they do not want to pay for them, yet, they will hire an agency to get staff and maybe or maybe not, they will keep them. The politics are getting ridiculous and while I find that I'm having to redo my BSN over again courtesy of UOP, I do not see a rosy future for the patients that medicine is supposed to serve. Tim equated a patient, a person, with 3 diagnoses. Well, Dr. Tim, I can have 10 patients with these same diagnoses and they all present differently and I'd better have enough assessment skills to know when the first sign of trouble shows up to head it off at the pass. I have seen patients undertreated at city hospitals, even those who have stellar reputations because they don't want to take the "hit" for the "frequent flyers." This isn't the practice of good medicine. I have seen this happen time and time again. There was the man with Alzheimer's disease that had a BS of greater than 500! He was quite rowdy, which is what happens when these patients have high blood sugars. I sent him to the hospital and within 2 hours he was back again, and they did absolutely NOTHING to bring down his blood sugar. Why? They didn't wnat to deal with the behavioral issues and challanges that these patients can present clinicially. You asked what I have achieved. I have achieved miracles. I have healed up wounds that nobody for 10 years could heal up. I have saved more people than you can count and it's all part of what I signed on when I became an RN, but the way medicine is going where you have to be part of a big behomoth whose only raison d-etre is the bottom line? That's not medicine and it's not nursing, it's just business as usual and its the patients that are getting the shaft.
