I wont undergo any treatment if ever this anal cancer will come from these toxic steroid heamorrhoid creams, i consulted doctors for prevention of AC and they increased my AC risk with these evil creams causing anal cancer!!!!!

Yes, these next two slides are indeed exactly why I came here. This worked so amazingly well, that I have been able to learn how to love myself in a way that I had never knew I needed for 35 years of my life. All I want to do now is better understand this mechanism and all of the related mechanisms so that I can do what this substance has done for me, on my own with my own brain chemistry and exercise and life and diet and everything in between. Absolutely amazing stuff! Such a great lecturer! Thank you so much for this!

Speaker is wrong. He says you dont need to many, if any tests? You need to do LOTS of tests. You need to rule out parasites, bile acid malabsorption, candida overgrowth, drug use, etc etc.

I can't thank you enough for this outstanding post. I am a gynecologist myself. 65 years and have undergone Total colectomy with removal of rectum, J pouch and diverting ileostomy for adenocarcinoma sigmoid colon developed in a long standing UC. This was done 5 months back. But when I was posted for ileostomy closure my distal loopogram showed a leakage of dye from the anastomotic site at the junction of anal and J pouch. What should be done. I have no symptoms. Should I opt for permanent stoma or can I get my closure of ileostomy after 3 months ( as advised by my surgeon). I

If you get an invasive fungal infection the medical system will leave you to die from it. The great doctors will do nothing about this infection. The whole medical system and all the doctors including the doctors who work for Infectious Disease Control and Prevention left me to die from this.

Introduction - Follicular lymphoma is an indolent B-cell non-Hodgkin lymphoma that often follows a chronic course, with recurrent remissions and relapses over many years or decades. Median overall survival exceeds 20 years. - Historically, a "watch and wait" approach has been commonly used for asymptomatic, low tumor burden follicular lymphoma, given the long natural history. More recently, initial therapy with rituximab or rituximab chemotherapy combinations has been increasingly used. - There is debate over whether immediate treatment or watchful waiting is preferable as initial management for advanced stage follicular lymphoma. Potential advantages and disadvantages exist for both approaches. Arguments for Immediate Treatment 1. May enable longest treatment-free intervals - Treating lower risk patients (by FLIPI score) earlier could theoretically provide longer remission duration from initial therapy. - Early rituximab can delay need for chemotherapy for over 5 years in some patients. 2. Treats patients when healthiest - About 25% of watch and wait patients experience health decline over time (infections, anxiety, other illnesses). Treating when healthiest may be beneficial. 3. May improve quality of life - Watch and wait associated with anxiety/distress in some patients. Early rituximab showed better quality of life compared to observation in one study. 4. May reduce transformation risk - Meta-analysis suggests early rituximab may slightly reduce transformation risk long-term, but data inconsistent across trials. 5. Could reduce clonal evolution - Early treatment may theoretically limit further mutation acquisition and evolution of high risk clones. However, no clear evidence this occurs. 6. Possibly improves survival - One trial in relapsed follicular lymphoma showed an overall survival benefit with earlier retreatment compared to later at conventional thresholds. 7. May reduce costs - Modeling studies suggest upfront rituximab induction could incur lower lifetime costs compared to watch and wait. Arguments for Watch and Wait 1. Excellent long term survival with current therapies - Survival continues to improve over decades, exceeding 20 year median with rituximab chemotherapy. No clear survival difference based on initial management approach in trials. 2. No advantage for immediate treatment in low tumor burden - Three pre-rituximab trials showed no significant benefit from early treatment over watch and wait for progression-free or overall survival. Findings supported by recent rituximab-era trial. 3. Advanced stage disease observational data shows no survival difference - National Lymphocare Study: similar overall survival with watch and wait, single agent rituximab, or rituximab chemotherapy despite differences in progression-free survival. 4. Benefit of less toxicity - Avoidance of treatment side effects is an advantage of delayed treatment initiation. 5. Transformation risk may not be reduced - Randomized trials found no significant difference in transformation risk, contrary to findings of some retrospective analyses. 6. Preserves future options - Allows sequence of multiple lines of therapies over disease course rather than use immediately after diagnosis. Considerations for Treatment Decisions - Factors favoring more initial therapy: high tumor burden by GELF criteria, limited stage I/II disease, significant symptoms, high FLIPI score - Factors favoring initial observation: low tumor burden, advanced stage, low/intermediate FLIPI risk, patient preference - Early relapse within 2 years predicts significantly worse outcomes regardless of initial management Conclusions - Excellent outcomes for most follicular lymphoma patients, with median survival >20 years in rituximab era - No clear survival advantage shown for immediate treatment approach in clinical trials - Benefits and risks to both immediate and delayed treatment initiation - Treatment decisions should be individualized based on disease factors and patient considerations - Early relapse predicts poor outcomes and should prompt treatment at that point - More precise risk stratification approaches may help determine who needs treatment versus observation

I have to have a Stem Cell transplant towards the end of 2024 for constantly replapsing stage 2b Mycosis Fungoides T Cell Lymphoma.So far I am on Brentuximab to try to bring the Mycosis Fungoides down to a level where a transplant is viable.

What about a lower leg ?venous chronic Wound with significant pain due to neuropathy. Very sensitive to silver. Using BioPad with foam and compression. Patient still has serious pain for initial 24 hrs. What would you recommend.

I read by another specialist on here that you can have IBS without pain. I read that alot. And instead have just say, gas pain. So we are in limbo here. I think we are all different.
