Don’t even need to listen- yeah, MDs are, indeed, morons. The list is self serving, which surprises zero people that have ever met one which, unfortunately for the general population, is everyone. The two biggest problems with MDs are that they lack critical thinking/comprehension skills and they’re egos are out of control. I am truly convinced that this is by design. Those that are apt rote learners are not great at actual thinking. They stick, dutifully, to what they’ve been told for that all mighty pat on the head. This cycle continues and they begin to think they are brilliant and the keepers of health. They stop listening and start their all too familiar abuse- gaslighting, lying and the ever present, “I have no idea so you must be insane.” On top of that they have no concept of the history of modern petroleum based medicine, why they “treat” as they do, and of the which damage and death they cause via ineptitude, indolence, malfeasance and malpractice. Then they scream that there’s a trend to “anti-intellectualism” because they get called out for being wilfully obtuse. I’ll bite. There’s a trend alright, and they’re leading it. In the early 2000s when Israeli MDs went on strike the population actually became healthier and fewer people died. What does that tell you. Bless their hearts, they’re oh so special, indeed.

This report is not accurate. I saw an interview with Radanda, on television. When the lady died, everyone was trying to figure out what had happened. Radanda stated on television that another nurse who was looking around, found the paralytic bottle, approached Radanda, and said, "Is this what you gave the patient?". To which Radanda replied, "OMG, yes!" Radanda did not discover and report her own mistake; another nurse did.

I have done plenty of palliative extubation, terminal weans as an RN giving meds and an NP ordering meds. 2000mcg of Fentanyl at one time is really extreme, even for these patients. There are many combinations of medications to use together at lower doses to achieve comfort. Fentanyl and Ativan together, versed and morphine, so many different ways to make comfortable without extreme doses. And that is just horrible telling family members a patient is brain dead without going through the criteria.

As a critical care doc You have not proven anything Most doctors don’t understand how to allow the patient to pass with peace 50mcg is nothing You are not an icu doctor. Patients require an increase of many drips especially narcotics. Most doctors would have patients on morphine and die uncomfortably. When we go to high doses of 1000, you have to look at the entire picture. I would rather put a patient on fentanyl at high doses than let them be on low doses and struggling. Doctors including ICU doctors must be strong and ignore nurses and doctors who don’t understand the medicine Your patients would not require more than baby doses if fentanyl so stop comparing apples and oranges. Nurses do what doctors do that they have known for a long time. That is annoying because nurses can be mean and hateful and stick together even when they are wrong. If all family is present and palliative care is ordered; I am going to give high doses of what is needed. Pharmacy doesn’t use the drugs and question what ever is considered costly. I don’t care. My obligation is to my patients and not the ceo or pharmacy. No matter how you twist it palliative care is assisted suicide. Watching someone struggle for week or weeks because of a ridiculously low dose of drugs not understood by others doesn’t make the doctor wrong. Since you aren’t critical care or anesthesia you should stop this story . Your knowledge base of appropriate meds for patients and experience truly only allows you to say things that isn’t your skill set. The way you have presented the story sounds ridiculous. If you said he was pushing potassium on patients of 100mg then I would question the doctor. But these high doses are likely a drip form and not iv push with a patient without a breathing tube. That would be suspicious but if these patients are on a ventilator and high doses of fentanyl was given as a drip; they didn’t die froM fentanyl. The worst would be a frozen chest from giving the meds to fast. But you can correct with a succ or propofol and the chest will relax. Icu is the place were folks are kept that have a high chance of death. With what you have told us; the doctor should be acquitted. Generally, nurse band and complain about what they don’t understand and then tell doctors who are usually don’t have the skill set to actually analyze the patients care. The doctor based on the story you presented should not have had his license removed. He likely made enemies because of the ignorance of those around him. Pharmacy wants to play medical doctor. Very few understand medicine and what is needed. I just hang up on them. I don’t engage with ignorant nurses or doctors. It’s enough stress to take care of my patients. I am independent and I am so because when you put ignorant and discrimination together; I leave. But many doctors and nurses have written patient is brain dead. They were wrong. Standard for brain death is blood flow study. Two doctors agreeing means nothing. Each body is worth billions if the organs are donated. If people really understood what goes on; they likely would not go to hospitals or they would demand fewer cook books demanding doctors follow and require doctors to use their brains and not be punished from cook books made up by folks who don’t actually take care of the patients. Don’t spread what you don’t understand. There is no reason not to give high dose of most drugs except fear by those who don’t understand. But, people will give high doses of propofol drips, patients develops propofol infusion syndrome; which is not reversible because you just killed the heart. No Pressor will help. The patient looks septic but isn’t. You just killed the heart with propofol. I am board certified in internal medicine, anesthesia and critical care since 2000 I have focused on the ICU but i practice all three because the combination gives me a wonderful skill set to take care of patients. People will bother me but not the guy who is over the ICU and killed two patients because he couldn’t intubate. I call it loyalty to stupidly. Come at me if you want I won’t respond. I write hear only to ask the doctor not to put out what he doesn’t understand. You aren’t giving 50mcg of fentanyl to any adult without other drugs and sedating anyone. You are either using other meds with the 50 mcg alone or just helping the patient to relax. Fentanyl kicks in 2 minutes, peaks at 7 minutes and last two hours And you can reverse with narcan but remember narcan only last 30 minutes . You must consider the amount of time needed to reverse If you just gave fentanyl , you will need to either start a drip or redoes narcan. Not trying to disrespect you but educate you and audience. The pharmacy makes the most money and can loose a lot of money. They bug me but I could give 2 Rocks I will always fight for the patients despite those around me. My strength comes from GOD. My medical oath i take seriously. I end up working free a lot but i don’t care. If I need help, Iam not to proud to ask. If other icu doctors need my help they will let the patient die first and no one says anything. Based solely on the information you provided; this doctor seems weird but you have not presented a reason for his license to be removed or that he has done anything wrong Sounds like enemies and bully’s using their power . So the more we patients (payers of insurance premiums and bills) pay the less medical workers get paid. Okay. Why is that? Patients are doing their part and even having to file for bankruptcy in the process. Medical workers, especially doctors, do their part and get paid less so they are overworked and give poorer quality medical care. We patients pay more and more to get less and less; medical workers get paid less and less. Where is the money going? Corporations. Private Equity. Insurance industry. Pharmacy benefit managers. Big Pharmaceuticals.

You are bombarded by a.bunce of sick people spreading all sorts of disease too make it worse Thay make sure you sit there for hours then Thay put you in a small waiting room too make sure everybody with different diseases sit in the same room and sit in the same place touching the same things then you must come back in two weeks so little Johnny with his snot nose and diarea that plays with little wooden blocks and magazines can get another chance too expand the deceased confined room ,I go to my vet for advice , Doctors don't care Thay just see money
