I was a bit nervous on my first cataract surgery hearing I would be awake to assist alignment of the IOL lense. With the eye numbing drugs & covering my non surgical eye it was remarkable that I had zero pain & awareness of the surgical instruments used during the surgery. I was quite tipsy heading to my return home car ride. The most irritating thing was the eye drops used before & 3 weeks after were somewhat irritating for a minute or two each application.Some of the post surgery vision effects for a couple of days were pretty wild especially my day two walk home from the park where I saw two complete sets of car lights & some weird temporary halos in my vision.

Thank you for producing this series on light adjustable lenses. The videos were very informative as I began the process shortly after you did. I'm now 9 weeks since my first procedure, and will have my 2nd and final lock in this coming Thursday. It was quite an amazing adventure and for me very successful. After 78 years of wearing glasses (bifocals or trifocals since being a senior in high school), I now am able to experience 20/15 vision distance and intermediate and near are crystal clear and sharp (J2). I feel quite fortunate I received these results, and I had not really expected them. I wish you well on your procedure, and look forward to hearing your final result becomes all you hoped for.

I was wondering if you have noticed any reflections coming from the lens which are visible to others? That's a minor concern as the important thing is to see well, but I would prefer that reflection not to be present. Thank you so much!

The lens is completely transparent to anyone looking into my eyes. No one will ever know you have it in there unless they are looking with a medical instrument like a slit lamp used by your eye doctor.

I advise you to think Binocularly and not Monocularly, in the sense that your both eyes should complement each other and not act solo. I mean put astigmatism aside, which should be corrected no matter what, you will not be able to regain a 20/20 vision acuity in each eye separately considering many factors, including age. The eyes should be working hand in hand to deliver a good quality of life, taking your lifestyle requirements into account. In other words, the eyes should be responding to the needs and wants of your life, whether you are a retired person, an active person, a working individual with screen exposure in most of the times etc...Blended vision strategy is best.

I had my first treatment last week and like you I don't see much change at all. Near requires reading glasses. Mid (computer monitor) is horrible. I have to put my face right in front of the monitor with readers. So far, I'm unhappy, especially because I feel I was misled. Curious as to where your second treatment video is.

No second light adjustment yet. Trying to work out some issues with starbursts and double vision. Until my doctors and I have a clearer understanding of what's going on, I've put further adjustments on hold. I think patience with these lenses is often required.

Thank you for your feedback. Very helpful indeed. My first surgery (right eye) is scheduled for this Tuesday. Wearing contacts on my left eye. Honestly, I am more anxious about the adjustment phase rather than the actual surgery. Worried about whether my last adjustment may be worse than prior adjustments. Wondering if the adjustments should be based relative to my left eye or totally independent. Have you had your second adjustment and if not, how is your vision thus far?

No second adjustment yet. Still working out some issues related to double vision. But my astigmatism in that eye is virtually gone and distance is excellent. I only had cataracts in one eye but suspect getting them both done around the same time is the best way to go so you can adjust them for perfect balance. The best of luck to you!

@@LKearns I'm in same boat regarding my other (left) eye. As far as I can tell, cataract has not progressed far enough to impede any noticeable clarity of vision. But now I'm wondering I should just go ahead and get it done right now to balance out the vision. I'm was thinking of asking the doctor to adjust the right eye for intermediate to distant vision and use my left eye for near vision. Currently wearing contacts on my left eye so am prepared to adjust the prescription if needed to focus more on near. Let's see..... Hoping all the best for you. Will update you after I come out of surgery tomorrow. By the way, how did you feel rest of the day on the day of your surgery? Should I expect to be drained out or rest in either case instead of reporting back work (remote)?

I too am trying for distance vision in the right and will get the left more toward near. That is what I have had for the last 30 years after LASIK. For some reason my brain is struggling more this time. As far as after surgery, I felt great, although not quite certain I would have returned to work. Having said that, I did shoot the close out to my Surgery Day video two hours after the procedure. Fantastic! Unfortunately it looks like my starbursts are coming from HOAs/irregularities on my cornea that the lens can't correct. However, if I take Lumify drops prior to night driving, it's definitely tolerable. Good luck on your next adjustment!

Thank you for this video. I'm 63 years old, have used readers since I was 40 and am now getting to the point where I need full time glasses. I went to a recommended ophthalmologist to get Lasik. He told me that Lasik is not permanent and at my age, that my eyes will continue to get worse and I will need glasses again. He said lens replacement is permanent and I would never need glasses again and would never get cataracts. He then offered me a 3 choices for lens replacement. Monovision with one lens for distance and one for up close; Multifocal for both eyes; or LAL. I'm real bad at making decisions like this when there is no clear answer.

That is completely understandable. All lens types have their pluses and minuses. I had LASIK over 25 years ago and it was quite stable, but we all pretty much develop cataracts later in life and will need them removed and new lenses inserted. I urge to have some good deep discussions with your ophthalmologist (one who is very experienced in all different types of lenses) about what kind of vision you want going forward and how he or she thinks that might best be achieved. Remember, the best lens is the one you were born with so hang onto it for as long as you can.

@@LKearns Thank you for your reassurance. I do have have high level of astigmatism and I believe the doctor mentioned that the length of cornea is slightly longer than average. I'm just hoping the laser incision does correct my astigmatism but am not confident it will since the doctor mentioned he may still need to make post surgery adjustments to correct my astigmatism. Did you go for your second adjustment yet? I would be really interested in what your experience and thoughts were from that appointment.

No second adjustment yet. I want my eyes to completely settle before taking another step. As far as I know, the LAL is quite good at correcting astigmatism up to 2.5-3.0 D. If your astigmatism is greater than that, you are correct, your doctors can often do some touch-ups with a corneal incision or laser vision correction. Here's hoping for your best results! Keep me in the loop.

This was interesting. I'm getting my first LAL end of next week. I'm excited and nervous that I'll end up dissatisfied. Watching all these vids has taught me to temper my expectations. I have mild astigmatism and get starbursts and halos because of it. I was hoping that would go away.

Here's hoping for a very positive outcome for you. Remember, everyone's eyes are different, and mine have had several surgeries, were highly myopic, etc. In other words, they were complicated. LALs can be very good at correcting many astigmatisms. However, some may be very tricky and coming from an intricately irregular cornea, which the LAL may not be able to correct. But you won't know until you try! Another thing I have learned is to take my time between surgery and light adjustments if you're struggling. Give your eye and your brain time to heal and get used to the new vision/neuro adapt. Be patient. Keep me in the loop and let me know how it goes. I'll be sending you good thoughts.

Thanks for these videos. Question: did you get the RXsight LAL or LAL+ lens? I had the Panoptix implanted about 9 months ago in my left (dominant) eye and my surgeon is now recommending the LAL+ for my right because I’m very “particular” (his word) about my vision. He told me that the LAL+ lens is like the Panoptix, a multifocal, good at all distances with the benefit of being able to dial in the focus after the surgery. I’m a bit concerned after watching your experience. FYI, I’m mostly happy with the Panoptix with the exception of haloes at night and a gap in clarity at computer screen (or vegetable chopping) distance.

I received the RxSight LAL. As I understand it, many Ophthalmologists these days are using the LAL rather than the LAL+ for patients with rather complicated ocular histories like mine - LASIK with enhancements back in 1995, vitrectomy with ERM peel in 2023, etc. They believe it appears to react more predictably during enhancements. It's still too early to say what my outcome will be, but as of today I have good distance vision in my LAL eye (probably as good as that I can achieve) but not much near. Still havestarbursts, but testing reveals it's probably coming from my cornea rather than the lens. Problem now has been some diplopia or double vision. I had mono vision/blended vision for 30 years after LASIK without difficulty, but my brain is having trouble neuroadapting to the disparate images coming from my right eye and my left eye at present. A problem we are still working on before proceeding. Remember, everyone's eyes are very different and I think everyone's response to the LAL lens is different, but I still believe it was the right choice for me. The best of luck to you!

It's a monofocal lense. You don't get depth of focus in one eye.

You are correct in that the LAL is a monofocal lens. However unlike a standard monofocal, it is designed to give you some depth of focus, particularly if you trigger the special EDOF algorithm during your first light adjustment.

The starburst happened to me after my first adjustment also. It’s been a journey of six months so far. The starburst have not gone away, nor have they been able to determine the cause. When I chose to do these lenses, I could not find any negative feedback online. Thank you for sharing your experience, and I do hope they have a solution for you.

One of the most informative videos for LAL, thanks for the detailed, clear guidance. I had LAL six months ago on my non-dominant eye and had two adjustments. Still not locked in. With -0.5, my computer distance got way better but not good enough for the phone (around J4). My distance was 20/15 after the first adjustment to plano, but now it is 20/25 with -0.5, and the distance is not as sharp as it was after the first adjustment. You tried to avoid EDOF during the first adjustment, but you might still have triggered it as you moved toward the myopic direction around -1.75 (from +1.50 to -0.25). My understanding is that any myopic movement greater than 0.5 adds EDOF during the first treatment at the expense of sharpness.

Be the first adjustment 3. Target at least -0.50 or greater myopia… Since we targeted -0.25 we did not trigger the additional EDOF. However, any steepening does inherently increase DOF but not additional DOF. Hope that helps.

Be the first adjustment 3. Target at least -0.50 or greater myopia… Since we targeted -0.25, my optometrist/light adjuster who has done more than 1000 of these adjustments, says we did not trigger the additional EDOF. However, any steepening does inherently increase DOF but not additional DOF.

Just curious, are you a professional presenter? The production of your videos are great and the way you present is like a news reporter. I hope you starbursts go away unlike my experience. I think the LAL technology is too new and the videos out there are over hyping the results. If you don’t mind, I’m going to link your video In mine. BTW, I did contact RxSight about the starbursts, in short, they say that the FDA study showed the same rate of occurrence of visual phenomenon that was lower or equivalent to the control monofocal IOLs.

@@SharonSews Sorry to hear that. Last week, I got my right LAL replaced with a monofocal lens. WAY better. Indoor glare is practically gone. Starbursts outside has reduced from 8-10 to just 2 small thin ones. It’s only been a week so I am hoping there might be further improvement. I hope to publish a video about the replacement procedure in a few weeks after I see if things improve. I will say, though, that the replacement surgery was way more rigorous than the first cataract surgery and costs were not covered by insurance.

I just had my right eye cataract surgery this very morning. The left eye was 2 weeks ago. I'm hard pressed to even call it a "surgery" versus just a procedure. Painless. Fast. In and out in about 90 mins. I have had regular doctor appointments that took way longer than either of my cataract procedures.... Already dinking around on my phone....

Quite informative and thank you for posting. I went to a consult ( (2) with a cataract specialist in Alberta and the surgeon spent a total of 25 seconds talking to me. I guess that is what Soviet style medicine is like. Will check out the manufacturer web site to see if anyone in Canada does the LAl+

Thank you for this. I'm about 1 week behind you in the process, but had LAL in both eyes. Same dilemma as you, correct astigmatism and then what else? My doctors plan for me is to correct the astigmatism and achieve plano in both eyes in the first adjustment. Now I'm trying to figure out this option that the EDOF can only be achieved in the first adjustment? Can you point me somewhere that can explain that?

@LKearns I am on a journey, too. Panoptix in both eyes months ago. Terrible IOL for me. I had one explanted already and am having the second explanted today. When 1st was explanted, I was left with refractive error. Now, my vision is not clear at all distances. Hoping for a better outcome with 2nd eye or the next couple of months will be a struggle to see because I don't have glasses set for distances. They will work on that when vision stabilizes. Months from now, I will have Lasik touch up, and that should hopefully get me corrected, so I only have to wear reading glasses. Good luck!

I know it can be scary, but if your experience is anything like mine, you won't remember a thing! Even though I was awake throughout the surgery, I was clearly very relaxed and happy. And afterwards, I remembered none of it. Good luck! You got this! Prepare for a lovely nap.❤️

Great two videos of your surgery and day one Post Op. Being 75 years old, I will be getting both eyes done, right eye first, then 2 weeks later my left eye. Currently, my right eye is blurry looking at distant views. My left eye is somewhat normal but has Cataracts. I am going with the basic Mono Focal "distance" lenses because of insurance covers my procedure and these lenses. Premium lenses would be out of pocket for me and too costly. I don't mind wearing glasses, I have worn glasses all my life. Thanks for the 2 videos. Take care.

Good luck on your upcoming surgery! I'm sure you will do brilliantly. I too would've loved to go with the covered monofocal lens, but because of my earlier refractive surgeries, it's too difficult for doctors to choose the right lens power ahead of time. Thus, the need for adjustments via the light adjustable lens. But I bet you will do great! And you're right, I too have been wearing glasses since childhood and will still need them moving forward but what else is new! 😃

Thank you Lisa for sharing this video. I am also about to do cataract surgery and I am trying to decide if I should choose LAL OR LAL+. Can you tell us why you didn’t go with LAL+? Is your eye still foggy a week later? Any updates please.

New video will be out probably tomorrow with my one day check. One week check will be after that, but vision is definitely getting better with each passing day. Fog is absolutely gone and distance vision is getting better and better. We went with the LAL rather than the LAL+ because from my understanding for those with prior refractive surgery, the LAL is a bit more predictable, but I'm sure your surgeon can provide you with the details. Of vital importance is choosing the right LAL doctor and light adjuster (not just your everyday cataract surgeon). Find a surgeon/light adjuster who has a wealth of LAL experience and knowledge who can make recommendations specific for your eye (every eye is different) is invaluable. @@LKearns I am glad to hear your vision has cleared. How is your near vision? Regarding LAL and LAL+, I believe that they start them both at Plano and adjust them from there. My thoughts is LAL+ gives more EDOF and near with the center bubble.

You are correct about the LAL+ giving better depth prior to the first adjustment. After that, however, from what I hear, what you end up with in the end is virtually the same. I think because of the aberrations in the cornea of a refractive surgery patient my doctor preferred the LAL. But as I said, every eye is different and if you have a good surgeon/light adjuster, go for it!

Thank you for making this great video. I've had consultations with 2 different doctors and just don't have confidence in either for various reasons, most of which you discussed. It's a frustrating process but I'm learning a lot along the way. I appreciate you doing this.

Thank you for this video. I found you through an l a l Facebook group. Initially I went in and had my left eye done first and they put in a torque because I have a tear on my retina. It wasn't until I went through a follow-up that my doctor told me that he wanted to put an l a lens on the left side and that he was going to give me monovision which I absolutely didn't want. I got mad and lost trust in him immediately because this wasn't discussed up front When I started asking questions about the LAL he was vague. When I got closer to my surgery date I was trying to ask questions and let them know that I was not interested in mono vision and I'd actually rather have a regular toric lens. I asked if the doctor could come in and talk to me this was a week before my surgery and they told me he had gone home for the day. I wound up talking to a tech and I asked a lot of questions that other people had never asked them. Oh another thing after I talk to the tech, because they told me the doctor had left for the day I wound up running into him later in the parking lot while I was walking my dog. So basically he refused to talk to me and lied to me and told me that he had gone home Needless to say I canceled my surgery I went and got some glasses. I only have a 20% cataract in the right eye but when I do get surgery I'm going to go with the toric lens. I do not want to be involved in this trial. When I had my right eye done I could see clearly for about 2 days and then somehow the lens shifted. Possibly from me leaning forward. I was told I couldn't fix it with the yag surgery because of the tear in my retina. So now I have when I left. It's important to me that I do not ruin the vision in it. I've heard many people complain about the lights bothering them inside an artificial lights along with driving. I don't know why anyone would even want to take this risk when you can just get a Toric lens? I told my doctor that I wanted to be able to see close so I could see my computer I don't need to see 15 mi away into the mountains. I would rather have driving glasses and not wear readers. I appreciate this page so much and will follow and like your videos and listen to them. Just from belonging to the Facebook group I see that this is a Pioneer filled with this product. I feel like they should tell you that up front. My doctor was dishonest I too have a degree in Media and have done investigative journalism. You hit the nail on the nose when you said you feel like being treated like a number. My doctor also works with another surgeon that does the adjustments. Also knowing that this surgery is so new and it isn't even covered under insurance and I'm in a Facebook group where I'm looking at doctors comparing notes so everything feels like you're in a trial because you are if the technology is so new. My doctor also told me that the success rate was 95% and with torque lenses it was only 30 %> That's when I knew he was lying. You are going to tell me that you have had a 70% failure rate with toric lenses for the last 30 years on glaucoma patients? I decided to go to a regular opthamologist because I was getting frustrated. I had had my first I done and had waited 9 months to go in and get the second one done and I couldn't see I was driving without glasses they wouldn't give me a prescription. My ophthalmologist told me from what he had found out that 50% of the people like the surgery but 50% in did not. He said the big complaint with people seeing halos and and lights One woman told him that when she went out in the sun itself changed her prescription. She was already locked in.

I'm so sorry you had had to go through all this. And you make some really important points in your comment so I thank you for sharing it. Unfortunately, as you know, no lens is going to be as good as the one we were given when we were born, but let's hope we can each find an excellent doctor who is willing to work with us and find the best lens for our particular situation. Sorry! I just saw your comment. I went with Intrepid Travel on the ocean endeavour. As I'm sure you know, there are a lot of solid companies, but this one offered a solo cabin at a "reasonable price" so that was a key for me. I would love to hear about your adventure when you are finished!

A couple of the days were sunny. But many were overcast/cloudy or foggy or snowing or late at night early morning since the sun never really set. Actually many of the days were a combination of both sunny and cloudy. Maybe I'll make some more videos that highlight more sunshine but this one is very representative of what I experienced.👍
