Is there any plan to analyze the ductal versus lobular subgroups? Since lobular cancer is primarily a later recurrence pattern than ductal, I’m wondering if it will be discovered that lobular patients need to take it longer, possibly on the more manageable 100x2 dose? We’re there enough lobular patients enrolled to do a longer term analysis?

On the basis of decreased distant recurrence and treatment related failures at 3 yrs but no change in OS, how can this be accepted as standard of care. Some limitations are- 1.There are too many variables in the standard arm such as duration and choice of chemo. 2.Time to surgery from chemo-rad is different in the two arms ( 6-8 weeks in the standard and 24 weeks in the experimental arm) 3.Upper rectal cancers were included in the study whereas the role of CRt is not defined in such cases. 4. Higher conversion rate in the exp arm probably due to higher Per-op complications. 5. A double PCr (28% vs 14%) in the exp arm did not translate into OS. The compliance to chemo was much less in the standard arm compared to 100% in the exp arm, so it’s not an exact comparison. 6. Long term results will determine wether there is any increase in local failures due to short course CRt.
