Radiculopathy caused by synovial cyst impinging nerve - first injection didn’t relieve the pain - I’m now on my 9th month of pain which isn’t as acute as it was back in Feb when it all started - scheduled another injection and nerve block for next week to try and pop this cyst - any tips?

Hello gentlemen, I have been recommended for spine surgery. I have grade 1 spondylolisthesis, 8mm slip. The operation will be a posterolateral fusion at L5 / S1. I am 66 years old. It is not encouraging given Dr. Zhou's comment on the rate of failure on the pedicle screws. I have been researching this for some time now and all I see and hear is negativity for fusions. I am managing daily life with medication, however, I have nightly bouts of extreme discomfort, waking every two hours or so. I do not medicate for this. Throughout the day, there are times when I am completely pain free, however, I am taking morphine and Tylenol, both at the maximum dosage for relief. I realize you will not give a definitive response, not having evaluated me in person, however, is there any possibility of avoiding the fusion. I reside in Canada, the operation has been performed here for the last 25 years or so without much change in the hardware and/or method that will be applied.

I’m having L5-S1 replaced this month and it’s not for back pain and arthritis, it’s for nerve pain and compression that I feel specifically on the left side of my pelvis, hip and hamstring so this video actually makes me feel better to hear him back up the good reason for surgery for nerve pain

so by eliminating herniation or protrusion of the disc the facet joint syndrome is going to go away?
@gnuintown4 ай бұрын
I don't believe so, no, as any herniation or protrusion would reduce the disc volume and therefore the disc height - reducing the clearance or space in the facet joint. Surgical removal of the protruding or herniated material should reduce compression and inflammation of the nerve at the disc, but not address pain at the facet joint. I've been trying to treat pain from my L4/L5 and L5/S1 disc issues for twenty years. I saw a neurosurgeon last week who believes that my "primary pain generator" is actually from the facet joints. It will be great if he is right and I'm finally able to get on top of my pain... I just wish it was twenty years earlier!

Thanks for this session. The comment about psychology and mindset is super important. I got diagnosed with Grade 1.5 Spondy at L5/S1 last year. I experienced kinesophobia and sought out a McGill assessment. Now I know how to move better, and manage my pain. In my late 30's, so my spine has a decade or so before the tissues start to gristle over and give me more natural stability (late-40s/early-50s). My life circumstances do not allow me to have a bad flare up and be unable to function for my family. When life circumstances are overwhelming, and we don't have support...catastrophizing is easy and we seek seek out the certainty of a surgical intervention. I would love to see this discussed more in these sessions. I miss being able to live an active lifestyle, play with my children, and live without fear of pain and disability. Some days I want to pickup the phone and get surgery scheduled. Today is not that day.

For me i just started to really focus on core strengthening. That includes body squats and planks, stretching like a mad man both the morning and evening. I also changed my diet trying not to eat inflammatory foods like sugar. I seem to have a good handle on it but every now and then it will light me up with no warning. I refuse to get surgery. Fuck spondy!!

If you have back pain, you're not able to learn anything because you are in PAIN.
@thespinexchange8993 Жыл бұрын
I must admit those were my first thoughts when I saw the clip. This clinician clearly hasn’t been in the pain himself. That said, as someone with long term back pain and associated issues myself, from early 90s sciatica, to numerous micro-discectomys and then finally a recent lumbar fusion, I’ve personally had no choice but to try and see beyond the pain. Best wishes Steve T (SpineXchange team member)

After my decompression , and fusion in February due to severe stenosis at L3, I still have pain. Saw a neurosurgeon last week, his answer was a bio- stimulator...I don't believe it's the only option...he also saw that there is movement in my new fusion after x-rays at different angles and xray with major arched back..was really sore. Bio stimulators, what is the success rate? Is it too early, 9 months, to be thinking of that?.

This resonates with me. The quality of life question is a wait and see game after my diagnosis. There’s no way for me to know what to expect with my recovery after the flare up and diagnosis. Not jumping straight into surgery seems sane. But loosing months if not years of quality of life waiting to make improvements in function/pain is not easy
