Interesting about the notebooks ... I have a ring binder for each book and EVERYTHING is in there. It might be character outlines or a story sketch, then later I add a short summary of each chapter and write page numbers alongside - that way I can (usually) go back and find a particular part of the story.

I full deep point of view thoughts are just part of the narrative. Whether first or third person we are inside the character's skin so it's about weaving the internal dialogue through the narrative.
