I've had this happen before. It's something that happens when there's multiple collision errors. If I remember correctly, the way I usually can reproduce it is to make a cube sit inside a portal, then, take the radio and move as far as possible from the portal and try to shove the cube through the portal using the radio. If you do it right, the cube and radio's collision should both break. EDIT: This is called Item Save Glitch, and you have to do what I wrote above, but put the portal on a corner first. This persists across saves and even if you start a new game unless you first close the game entirely. On the other hand, this gives the companion cube level some extra challenge 😄

I'll try and replicate this! It would make sense since I placed the portal under the cube dropper before the cube fell so the radio and cube went through at the same time and from that point onwards the cube was screwed!
