"I took your pulse" was in the script. It is only the delivery and leaning into the shadowy side of Irene that was improvised by Benedict. He talked about it in a commentary and people mistook that for him coming up with the line. Obviously the line had to be in the script, it is a central plot element and the pulse-taking was filmed in great detail.

I thought that line was absolutely brilliant because it further established Sherlock's character... even in a seemingly intimate moment with Irene Adler he remains the stone-cold detective, analyzing her behavior rather than being affected by her attitude towards him. Never knew it was Ben's idea.
