Well, the problem is that it is actually our fault, not of the game. While in development, many players stated that they hate the percentage chances of hit/miss (since gamers these days are spoiled and want just pleasant outcomes in games) so Firaxis did a nice little trick by using a multiplier regarding the difficulty. In other words, the game displays 85% but in reality this guy has 60% or something around it. We see what we want to see and Firaxis gave it to us. This is why it rarely happens that people also miss on 100% since it is never 100%
