I remember some of my nurses from my time in the ICU. I cried sometimes but they were very supportive and believed my hallucinations. Of course they sound strange to them but when you’re in that state they are real to you. Hallucinations can happen with a mixture or dehydration, hunger and anxiety

Is there a way to get part time and prn jobs as a new grad? I'm fine with a regular orientation, but I'm caring for a sick parent at home, so a full time, rotational job isn't right for me.

Such great information! I was an L&D Nurse for 7 years until I totally crashed and burned. I was fortunate to quickly find work outside of healthcare but swore I was never coming back to nursing. Healing didnt happen overnight, I really struggled with the loss of my career and severe depression. It took over a year to finally feel like myself again. I'm finally ready to return as a travel nurse this coming January. Podcasts like this are helping me prepare to return with a new mindset, new drive and new energy.

I am in my first nursing job on a med-surg unit in a small rural hospital. We don’t have CNAs, and we draw our own labs. Many days we don’t have RT either. I’m in my 5th month, and I’m FINALLY starting to get a handle on the stuff that CNAs would do in bigger hospitals-occupied bed changes, being efficient with getting vitals on everyone, giving bed baths, toileting, etc. Talk about feeling unprepared! On a normal day, our unit has two nurses, so that means when you need help with a two-person task, you often have to wait because the other nurse is with her patients. The other thing is I’m a new nurse at the ripe age of 52. Patients tend to assume I have authority and experience, and explaining that I’m still pretty new and learning can be just weird. Lastly, my coworkers are much younger. We’re only just now starting to feel comfortable. I’d love it if you could address one or all of these issues at some point. I love the podcast. Thanks for what you do.

Not Only assertiveness by the ability to not show when you are bothered even better the ability to not be bothered in the first place. Instead, keep your mind set that you’re there to learn, and they are not going to get in the way of that. Let them have their attitude say whatever snarky remark, but you continue to follow them like a shadow and learn. just get it done. most of the time people who bully other people do it for the reaction. If you don’t have a reaction, it literally deflate them.

The Problem is i have Severe Needle Phobia. as a child i Refused all Vaccines and Hurt Doctors in the process. back in 1976 i had 8 doctors try to force and hold me down to give me all my vaccines i destroyed the medical room and hurt all 9 Doctors as of today i am vaccine Free. i will never step foot in a Medical building. im not worried about these diseases i had them all and im not dead. as far as covid type o blood is immuned to it from what i have been reading. Needle Phobia patients will not Even show up at These Vaccines places its not going to Happen.

Good points. Learned during my first summer job in a pretty toxic working enviroment that pointing out malpractice to an older colleague can be quite challenging and have to be handled with precaution. She even saw the results afterwards when the patient passed out. As a result I just didnt piss off bitch A but also B and C when she started spreading rumours about me for being lazy and impolite. It was like returning to Junior high. You don’t see that in white collar working Sets. Thats why we use anonymous reporting channels so we dont have to worry about getting into a cat fight with veterans suffering from poor accountability.

It sounds like a CNS only works in a hospital then? Do all CNS have prescribing ability?

From my understanding, they all do. However, I am not 100% sure. It sounded like from the discussion that most work in acute care. The degree does provide graduate-level education, so there are also a plethora of opportunities in nursing that you would also be qualified for.

I start as a nurse manager (first manager role) on a busy IMC/ICU Step-Down on 11/11. This floor apparently has a history of the seasoned nurses (those with 30+ years of experience) causing drama, not respecting their leaders, etc. Do you have a video or episode for new nurse managers?

I used to work on a unit like this, and the informal leaders who were bulldozers tested the new nurse manager with what they could get away with at the beginning. If you walk the line of being friendly but not friends, holding accountability consistently, while also giving them praise/encouragement/positive reinforcement - that should work well. My recent live on commanding respect vs. earning authority might be helpful too. I'm guessing you may have some who think they know more than you, but being a manager is totally different than high level clinical skill. so you'll want to command respect, knowing it'll take time to earn authority. There will be things they do have more knowledge about (operational daily unit things) but there will be a lot they don't know (budget needs, regulatory requirements, etc). So tapping into their knowledge base ("Hey, I'm curious about how XYZ has been done in the past and how that's gone...") can open the door for a way to value their input while keeping it in their lane. Also maintain a physical presence as much as you can and take time to get to know people, even if they're known for being rough around the edges. Those experienced RNs hate managers who barely talk to them and are in their office all day, so just some face time and asking about their perspective can go a long way. Good luck!

Important tip to add: If you're the situation she's described, it is important to your best to navigate unit drama while also respecting the authority of the nurse manager to maintain a united front. The newer manager is still building credibility and it is helpful if as a charge nurse you make an effort to do your job well without stepping on their toes. So that can look like keeping them in the loop of challenging situations while doing your best to handle them autonomously, and not throwing them under the bus. If you're the head coach for the shift as a charge nurse, the nurse manager is like the Athletic Director. They're not in the game/at the bedside, but they're overseeing the operations. You want to do what you can to build a strong and respectable program without undermining them. This support should be reciprocated by good leadership.
