So none of this would apply to a 20’ fixed ladder? Cage is/will be compliant?

So I've just been assigned the job of monthly Ladder inspection, & everything is fine, but there is this one ladder I have questions. All other fixed ladders have 3 points of mounted, but that one is only mounted on the top & bottom with the mounting bracket unattached. It's very jiggly & if it fails the user would fall off a roof.

That definitely would be cause for concern! The ladder would need to be updated so that it's secure and safe. If the ladder is updated AND is over 24 ft., it will also need a PFAS (personal fall arrest system) installed to meet OSHA guidelines. Any ladders installed after 2018 or UPDATED after 2018 need to meet the new ladder requirements.

1910.23(c)(1) Rungs and steps of portable metal ladders are corrugated, knurled, dimpled, coated with skid-resistant material, or otherwise treated to minimize the possibility of slipping;
