Well done, way to flex that 550lbs! I’m a former ski patroller, mountaineer, and presently work as an arborist. I’m being recruited to my local hill’s SP. I found this video after browsing a F3 self evac kit made by Sterling and sold by Cascade Rescue…I did not find any video demonstrating the F3, but I gather it uses a moving rope system with a fixed anchor. F3 uses a very thin 5mm rope, and a fixed eye termination (bend radials too small for safe knots?) I like the simplicity of the stationary rope demonstrated in this video, especially that the group can save time & tangles by sharing the same rope….However, we should be aware that the belay/rappel devices used in this video are not likely to be compatible with new 5mm ropes now being sold with evac kits (Sterling F3). One solution for maintaining compatibility of methods and rope diameters is a new belay device by Grivel - Scream, which can bridge the compatibility gap of 8mm and 5mm ropes we all carry now. Also, if slacked out by the first, a second patroller could descend after the first on an extended F3 system. The drawbacks of Grivel’s Scream plate are an extra carabiner and a bunch of incorrect ways to load the widely adaptable device (needs more training).….All things considered, I think y’all nailed it! I’m very satisfied with this video! The webbing harness seems like the bottleneck, but I don’t’ have any better ideas…. Good mention of paracord, I survived a bare-knuckle paracord rappel in my teenage years. Great work!

Edit: if using a belay plate to follow a first on the F3, first have the first somehow secure both ends of the rope.

A fracture with angulation or deformity can be realigned without an x-ray. While the conventional mantra in the front country (urban, short transport to an ED) environment is "splint in place unless there's evidence of neurovascular compromise," this is not the case in the prolonged transport / austere / remote / frontier environment where misalignment increases pain and the risk of injury. That being said, even in the front-country setting, there is no significant downside to reducing a fracture / fracture dislocation before an x-ray. All the pre-reduction x-ray tells you is exactly how angulated and displaced it is at the time of the x-ray. It tells you nothing about the maximum angulation because no matter how well it's splinted, the fracture is likely to have moved prior to splinting and even post-splinting unless you are using a conformable splint. The final x-ray is sufficient to confirm the fracture and to guide an orthopod as to whether or not operative repair is needed. Having given myself an angulated both bone distal forearm fracture since I made this video, I would add the following: if you can do a hematoma block, do it! I asked my colleague to do one and once it was done, I was pain free for 6 hours. Although I still needed surgery later, he was able to manipulate it into a reasonably aligned position for the splinting and it made a world of difference for the pain control.

Wow, watching my hives inflammation fade day after day until they completely vanished was such a relief, I went with what I pointed out and within the first 10 days or so the urticaria/angioedema was barely visible! Another intervention during the sorting might be to give a couple of rescue breaths to a child or infant in arrest. While not stopping to do full CPR, a couple of rescue breaths to a child or infant takes less time than putting a tourniquet on an arterial bleed and can make a difference since so many children go into cardiac arrest because of respiratory issues (according to AHA BLS). Might be something to consider.

12-20 is considered normal, however, with triage, over 30 puts them into the immediate category without going further. Less than 30 moves you on to check capillary refill. Over 2 second capillary refill puts them into the immediate category without going further. Less than 2 sec capillary refill moves you into the mental status of the victim. If they can't follow simple commands this moves them into the immediate category, if they can, they are in the delayed category. I think he did a good job explaining the process.

nremt says 12-20 is normal. 20-28 is high but can be adequate. anything over 28 is considered inadequate ventilations so BVM is need it. but for the triage algorithm they chose 30 as the pivot point.

Hi sir. Question on terminology that I’ve always been confused on. I’ve seen this optimal head position called sniffing position. I’ve also though seen a slight head tilt chin lift in a pediatric or young child called sniffing position (aka not a full head tilt, rather tilting just enough to sniff). I don’t though think this pediatric position requires the head to be anterior or jutting forward. These are two different positions with just overlapped terms right?

wouldn't a water knot be better than a square knot? a water knot is the only safe knot to tie with webbing as far as i know.

Text books will tell you the water knot is the only safe one. Real world application is different. Have repelled with a hasty harness that was tied with square knots numerous times with absolutely no issues. Whatever your preference ends up being, just train with it.
