Thank you for this very interesting input - it would be very helpful to have such a marker for objective evidence to avoid being missdiagnosed with Cushing. I was being diagnosed having an iatrogen cushing due to weight gain, resulting from being too exhausted for sport activity, and that also includes the face and abdomen, but mainly legs and arms. Needing to discuss that the weight gain is not the only indicator for Cushing, and that its common to see weight gain in the face of a person and also at the belly, as its uncommon to gain weight only in the arms and legs., is very frustrating. Esp. when most of the other criteria for Cushing, that are esp. more objective (Weight gain can also be objectified by measuring circumference, if one is interested in making a correct diagnosis and to avoid the risk of a misdiagnosis) are not fullfilled and rather indicate cortisol deficiency. It would be thus amazing to have objective parameter for diagnosis, that are respected by doctors. People are not machines, where everyone needs the same dose, but are very individual in their metabolism - metabolism always sounds very simple, but it is a highly complex matter, with genes being switched on and off, as you have shown very nicely, and which then in turn can influence themselves directly or indirectly and are also influenced by other hormones. A very complex thing, but which makes medicin that fascinating. But It should therefore be clear that each patient must be considered individually - that should actually be self-evident, but unfortunately it is not. But such data as presented will help to make this complex situation clear and will be very helpful, if an objective parameter will be found. Thank you for the very interesting details 🙂

Great, I pray, that this is a helpful guideline and will hopefully help, that endocriologist will not harm patients or rather nearly kill them because of a diagnosis made easy, taking into account everything that fits into their picture and esp. based on subjectiv assessment and ignorring everything that is not, even if its more objective criteria. Playing around with a patient's life is very serious, not for the doctor, who simply stays calm, while the patient is fighting for life and requiring years to recover. No endocrinologist will be helpful for recovery, after they almost kill their patient, and thus it is better to avoid them, if one values a persons life. That sounds bad and it really is, although I have only had such experiences with German endocrinologists and they was much better with non-German ones. I would never have thought that it is not luck to survive something, and that medical treatment can have such serious long term consequences. I therefore hope that the guideline is helpful and for the benefit of patients, although doctors still need to think - if they only think purely in a schematic way, as is unfortunately almost standard in Germany, you sadly get better recommendations taking into account differential diagnosis from AI. Unfortunately, I fear that things might get worse for potentially affected patients - in such case and as a patient I can only hope that the next doctor, to which I have the courage to go (and if I will overcome my panic about doctors), will accidentally kill me and I will not survive the intervention or rather the lack of it, instead of having to live with a long-term damage like the last time. Even more so when you are laughed at by the doctor, because of worries and you later have to realize that the theoretical worries have been far exceeded by reality. But that is no longer the doctor's problem, as of course anything goes well and what they say is like a magic spell, and if it doesn't work its the patients failure and they must then see, how and if they can get out of the health dilemma caused by the doctor. The problem as rated by the doctor is still the wrong imagination of a patient, and a self-fulfilling prophecy in terms of worries - problem solved by sending to a Psychiatrist. In any case, I am very excited about the results and the new guideline, hoping to be helpful for doctors as well as esp. patients to receive the needed help.

If caveolin-1 is produced by the cav1 gene, and problems with the cav1 gene or cavin-1 gene, like a mutation are the cause, couldn’t they use gene therapy to replace the bad gene therefore restoring the process of creating the protein? I’m guessing it might not just be the gene but also something else along the pathway that could go wrong at any stage of the process of creating new adipocytes. But if it’s the gene couldnt they develop a gene therapy for that? Also, where can I learn more about this stuff, no one I talk to in the medical industry knows anything about this, is expression the same as your levels or is it more to do with other abnormalities with the protein. Also, where or how can I get my caveolin-1 levels tested to see if there’s a deficiency or abnormality? I have lipodystrophy but it’s to do with scleroderma, I’m sure, and I need to prove it’s what I think because I think that this all comes down to that protein but I can’t get it tested to prove I have an issue with it.

@subhankarroy9466 before surgery calcium was mild low parathyroid hormone was mild high to raise calcium in blood I made analysis after a month of surgery parathyroid hormone is in normal range despite calcium is mild low and vit d3 is insufficient parathyroid hormone in this case should be high .. but it can't..this is called " parathyroid insufficiency " I now 2 months after surgery I think either calcium is normal by supplement..and we don't need high parathyroid hormone or parathyroid return to its activity and its hormone is now high to maintain calcium in blood in normal state

@@ghaly46 sir...... It's been 19 days since my surgery and my parathyroid is still not working properly now my PTH evel is 5.79 Sir is there any chance in future that my parathyroid will work normally again I am very stressed, I have to take 500 mg 6 calcium tablets daily

QUESTIONS Challenging conventional knowledge! Surely endocrine sytem disorders are holistic all sytems imbalance ie poor sleep ,nutrition or no regular propr exercise will make hypoparasymptoms worse ,so yes I'm having my parathyroid levels checked but the hormones are body brain lifestyle dependent ,could one say lazy parathyroid I am against all this hyper rushing to surgery then the patient is hypo ,we need more out of thfe box thinking,like I hear USA surgeons amputating a long covid girls legs ,a gymnast this IS ALL f..n crazy ,let's take a step back and retake control ov our lives😢❤

Hi - I hope you can give me an advise? I got a thyroidectomy back in 2016 and straight after I had symptoms of post Hypoparathyroidism - it is also in my journal and I was treated with a little dosis of calcium (pill) for a while… But no one really believed me or my symptoms. Through the years the’ve gone really bad, but all the doctors think it’s a mental problem - which is really frustrating… No one will give me a real psysical check and it’s nearly impossible for me to get blodsamples… I’m a patient in Denmark and it seems like they don’t have knowledge about postsurgery Hypoparathyroidism - like it’s not a thing or not excisting? How can I explain it’s real? I’m getting so much worse all the time - and time is slipping away from me, which is so frustrating… What can I do?

@@sigma9050 yes. I had two glands removed and straight after my journal says, I had Hypoparathyroidism - it was back in 2016. But since then it’s been impossible for me to get a serious check up - no one wan’ts to see me - and in Denmark you have to get the doctors to send you because we don’t pay our self…

@@kristinaviktoriaditlev9089 omg , I am so sorry they don't believe you. I also had thyroidectomy and with that also a parathyroid gland removed. Here in Germany doctors checked me thoroughly and know what I have, but can't solve it. Since the parathyroid hormone is not doing its job, I am in Calcium acetate and Calcitriol (active form of vitamin D3). They said it may be permanent.

I've been doing my own trial (N=1) and preliminary conclusions are that for skin disease, topical application (D3 in an oil applied to the skin) works a lot better without side effects than oral administration. You probably can bet much, much higher vD concentrations in the skin this way (where it is needed) without driving up blood levels to points where you suffer from side effects (sleep loss etc.)

Big issue with the registration this year. After several weeks from paying by bank transfer ESE is asking me to pay again....really guys, it sucks...you really have a problem with the staff and with the understanding of such a simple issue, once the payment being performed by bank transfer this cannot be also be performed by credit card...it is the worst experience ever had with payment fee on a Congress...

Yoo, I have hypo parathyroid ism (idiopathic.) Just diagnosed, age 12, for no freaking reason 😅 I had no genetic causes or surgeries in my parathyroid. I just had anseizure at my friend's bday party. I take Tums (Calcium) and a medicine called Calcitriol. This is cool that these videos exist!

Very interesting. I have started taking a D3 supplement (I know I was borderline low) and have suffered with IBD most of my life, which is autoimmune. The one thing I will comment - empirically Zinc supplementation definitately has a big effect on colds and chest infections; I have been taking 15mg/day for past 12 years.

Take a look at who funds these people first. Yes, it's those who see vitamin D3 as a threat to their profits. Also, even I know high monthly bolus doses of vitamin D3 are unless.

I think one of the issues with the trials is the large infrequent dosing used. Daily dosing may be more effective.

I read that daily is best but weekly was almost as effective and monthly wasn’t good at all. Then they’d do insane ones like once a year which is terrible.
