In order to get the achievement you have to ESCAPE at the campaign's finale. Aka made it out alive, unfortunately you did get downed and that's why you didn't get the achievement...

Never stopped playin, glad you’re making content for it still! This map was a welcome addition, and this mode is definitely a challenge. I find I have to be shooting more or less non stop to have a chance 😂
