I think your count is off by a bit. Each row is meant to have stitches in multiples of 12. Row 1- 12st. Row 2- 24st. Row 3- 36st*. Row 4- 48st*. And so on

The hats are always too small for an adult and you can see the stitches stretching when put on. I’m not sure what I’m doing wrong. Tension is medium tension and if I go for the looser tension, I lose stitches. I’ve tried multiple yarns
