Very informative video.. thanks a lot Doc..🙏 But if David's partial tear of suprasspinatus was result of a sudden fall which affected a pre existing (smaller and of degenerative nature ) tear marking it now a bit wider, lets say 18 mm after accident, but still its a partial tear.. would tou suggest that an early surgical repaire should be considered as first line therapy? What about sports injury SLAP tear?? I'm 38 y, I have very strong shoulder before I injured my right shoulder by doing sports, after I injured my shoulder. It's really hurts every time move my shoulder . Now almost 10 months, I feel much better, I can move my shoulder about 90 ~92%ROM. but the pain never go away.. Some times it hurts. Some time less but still a little bit pain... Do you think surgery is worse in this case??

The best advice I have heard so far, on this subject. “There is no problem that surgery cannot make worse”. Cutting open the human body, putting screws into bone. That’s a big deal!

Hi, i am in 43 years and master of martial arts, i got knee arthritis in my both knees, before i run 10 km under 50 min and do high intense cardio beside weigh training, but now i so sad about future and chronic pain, my knee almost no pain after gel and cortisone injections. I am concerned about my daily activity, can i walk a much as i want, can i run without after knee pain and involving swelling! I am confused, dr david in KZbin says not use of your quad it makes your knee kills, just focus on your glute but you mentioned pressure on knee not worse cartilage and the process of disease makes it worse. Hello Dr. Luks! I noticed you didn’t discuss root tears. I imagine they always require surgery especially in order to avoid bone necrosis? Is no weight bearing required for 4 weeks post surgery? I am 79 and have had some arthritis for several years but manageable. However 3 months ago overnight I got in both knees terrible pain day and night, some swelling and redness. Creams, anti inflamatories, ice or heat, nothing works. I realize now that for 4 weeks beforehand, several times a day and 4 weeks duration I was sitting in a very tense position with my toes holding me still and holding a sick cat for sub fluids, not moving at all. The poor cat died and I am left with never ending pain. Doctor will think I am crazy but my gut instinct tells me this is what has caused it. I will overcome this phase I am determined.

HELP!! After 10 monthes with Plantar fasciites i decided to get some cortison injections. After some weeks without pain i went back to the tennis court. So i ruptured my plantar fascia. After another 9 weeks without sports i am hesitant to start running. My question being: are these side effects of those injections reversable? Or is this area forever prone to injury ? Dont wanna be scared forever.

had a torn achilles several yrs ago. it was very painful. made walking difficult all the time. one day..I woke up..and it was gone. Healed completely. hasnt hurts since. I say that was an act of God. BUT NOW, my other one is slightly torn. not near as much pain as the other one but is bothersome for sure.

Hi Dr Luks: After an MRI, I was told I have a tear of the posterosuperior labum associated with lobular paralabral cyst which extends ito the suprascapular and spinoglenoid notch. Mild edema signal involving the involving the supraspinatus muscle which could represent denervation changes.Also inermediate to high grade chondral damage involving the posterointerior portion of the glenoid...all in my left shoulder. I've already had a shot (worked for two days then pain back). Yesterday I had something called Regenerex PRP (stem cell &Platlet rich plasma). Agonizing pain after the numbing agent wore off. The pain was a 9/10. Today it's been hurting all day. Doc says it may take 2 weeks for pain to go away and then another shot with another two weeks. I'm not waiting that long (who would). My life is awful now. I'm NOT going in for the second shot in two weeks. The pain wakes me up and is 9/10 at 2:30am. Major sleep disturbance. Who should I see and should I get surgery? If so, what kind is least invasive and won't have me laid up for weeks in pain. And I'm not allowed anything but Tylenol. Help! I'm desperate. 65 years old male. Thanks doc for your great video.

Great information! I have a very similar situation. I am 54 and have been running for 20+years. I had been training for a marathon but in early August experienced some moderate knee pain during a run. The next day it was fairly painful, so I took a week off. The pain started to subside and after 2 full weeks the pain was gone. I then started by adding a few short runs and eventually was back to running normally. A week later I was 4 miles into a slow-paced run when I began to experience some severe pain. The next day was a lot worse, so I scheduled an appointment with orthopedist and also got an MRI. MRI showed a small flap tear, so they scheduled me for a scope to remove the flap. But, in the week following I began to read more about the other options and decided to get a 2nd opinion, the other orthopedist understood my hesitancy on moving forward with the surgery and suggested maybe some additional reset and/or a cortisone injection. 1 week ago, I opted for the rest and cortisone injection and after just 24 hours I was pain free. My question is how long I should rest before attempting to get back to running? I just started doing some squats, calf exercises and balancing on the injured leg.

Hello Dr. Luks, I recently had a nasty fall and did some extensive damage, torn labrum, torn rotator cuff and also chipped a fairly large peice of my glenoid bone. With that being said the problem is that my shoulder will not stay in place. It has dislocated or subluxed 6 times in the past month just reaching up for things, very miserable. My surgeon is saying that surgery is my only option to keep it in place. What's your thoughts on this and should i get a second opinion. Please respond so i will feel better about my decision. Even if you got the surgery the functionality would be relatively short lived because eventually you would be back in the same boat since the meniscus loss area would be on the fast track of going arthritic. U be worse off than if you just rehabbed the knee. The only time surgery is needed is if the knee is locking up or completely buckles under your own weight also mass obstruction present that impedes functionality.

i have a pretty serious tear. Ultrasound Doctor says I have a few strands left. So I'm thinking I have an 80-90% tear. I injured it on vacation and didn't see the doctor for almost 2 weeks. in that time, I injured it some more. So now I have it in a boot splint and crutches. The doc says it will probably not need surgery, but he hasn't ruled it out yet.

I strained my left knee half a year ago, the the problem is not the same anymore, my main problem now is that my left knee easily swells up if I am on my feet for too long, my physical therapist told me that I have to get stronger gluts and hips but still getting knee swelling. Can this be arthritis or something like that? And what do you think I should do to treat my knee from swelling?

How long does it usually take for the joint to stabilize? I have a grade 3-5 maybe depending on the day. Just passed 2 weeks and started moving my arm out of the sling a little bit. Still feels like things are moving around in the joint. Bone on my shoulder pops up in certain positions. Doctor wants to do surgery, i wanna see how it heals.

A steroid injection is very effective to soften a subcutaneous keloid that is limiting muscle movement after a surgery. But the injection is placed around the keloid and not deep. Often only one is needed. Multiple injections are not recommended.

I had alopecia areata. Had 5 cycles of Corticosteroid injections at the affected area. Doctor also overdosed as effect was not showing up. In 2 months after the last dose one of my knee swell up. Upon MRI I found my ACL had mucoid degeneration and complete rupture. Doctor kept asking ke what injury I had, I said none. I just did normal cycling which I was doing for years. It took me almost a year of research to find out my knee ACL ruptured because of the steroid. Wish the cosmetic hair doctor had warned me in advance. I needed the healthy knee much more than healthy hair. 😢

Gluten may cause knee swelling. Gluten is in wheat/barley/rye....oats/corn/rice. Rice is low in gluten. Wild rice has no gluten. LDN may help block hard to avoid gluten. Ribs out of place may swelling the knee and whole side of the body. Glutamine/slippery elm may help the gut lining which may help the knee.

I wish the internet was around for my 35 years of Sports Medicine and arthroscopy career. I would have been able to follow you and be encouraged and or validated in my efforts to educate and support my patients that were too often sabotaged within days after I had spent an hour explaining why they didn’t need surgery. Patient leaves office informed, reassured and confident that they are being taken care of by a knowledgeable, experienced surgeon only to have there neighbor , chiropractor, friends son who is an equipment rep tell them “ you have to have surgery for that! “ All my efforts were for naught. I was discouraged and I could have been consoled by your videos/ twitter posts!

I have a question? So I watched this video and I’ll say this I awhile back I got on the monkey bars and I feel off because it hurt when I fully extended my arm so then I noticed it would hurt when I would extend it and it was like this for awhile maybe a year then it just started hurting bad lately and I went in I got a mri done and I have a clavicle fracture 1.2cm and what’s crazy is I don’t remember falling or injuring it? I have diabetes so I probably feel and didn’t feel it so since it’s been hurt for who knows how long? And it ain’t getting better and it’s actually hurting me more now would it be better for surgery? I’m 47 years old so I just need to get back to work and I can’t right now cause I’m in pain?

I totally agree... I had a complete full rupture of my ACL last Oct. (2023) from jumping off my horse. I was so scared at first, I was under the impression that I would have to have surgery. After MUCH research, I decided not to have surgery and see how things would go. I'm a firm believer in the power of Prayer and am super active. It was a bit slow going at first, but I'm SO pleased to report that I'm doing great. I can run, ride the Peloton, ride our horses and much more. I'm so thankful you're willing to put this very valuable information out there. It is so helpful & important. Thanks so much!

Dr luks great videos I’m having issues with my R shoulder 5 mm superspinatus and 12 mm infraspinatus just had my 3rd injection yesterday and my question is with the size of those tears do you think surgery is required?also physical therapy wasn’t much help. I. A hockey player and want to continue to play . Im 44 with complete acl rupture and meniscus tear. 7 months of therapy and In 80% normal. One surgeon said i need surgery or else TKR in years to come. The other surgeon said keep rehabbing. No pain instability so keep strengthening. Now Im torn. This vid helped

Tore my acl and mcl at 46. My Doctor recommended PT and no more basketball. Now 63, can run, bike, play tennis, no pain. Keep active and continue doing PT exercises to strengthen surrounding muscles.
