🎯 Key points for quick navigation: 02:00 *🎙️ Introduction and Context* - Importance and relevance of oligometastatic radiotherapy, - Introduction of the presenters and the outline of the talk. 38:00 *🔬 Definitions and Treatment Considerations* - Definition and types of oligometastatic disease, - Considerations for using radiation and surgery in treatment. 04:57:00 *📈 Evidence and Clinical Trials* - Overview of clinical trials and evidence for oligometastatic radiotherapy, - Summary of trial outcomes in lung, prostate, and colorectal cancer. 06:50:00 *🔄 Radiation Dose and Fractionation* - Explanation of stereotactic radiation dosing and fractionation strategies, - Discussion about safety, toxicity rates, and cost-effectiveness. 08:36:00 *🎯 Candidate Selection and Stereotactic Technology* - Criteria for selecting candidates for stereotactic radiation, - Availability and capabilities of technology and treatment sites. 10:43:00 *🧠 CNS and Bone Metastases Data* - Data supporting the treatment of brain and bone metastases with radiation, - Overview of associated clinical trials and guideline recommendations. 12:54:00 *🧪 Oligeometastatic Trials: Lung and Prostate* - Details of major trials like Gomez, Iyengar, and STAMPEDE in lung and prostate cancer, - Results highlighting survival benefits with radiotherapy in oligometastatic conditions. 22:18 *📊 MD Anderson Extend Trial Outcomes* - The Extend trial included 87 men with up to five hormone-sensitive metastatic lesions and evaluated the addition of metastasis-directed radiation to hormone therapy. - The trial found improved progression-free survival when radiation was provided alongside hormone therapy for oligo metastatic prostate cancer. - The setup involved different forms of staging with CT, bone scans, and PET scans. 24:10 *🔬 Stomp and Oriel Trials Analysis* - Combined analysis of Stomp and Oriel trials showed improved median progression-free survival with radiation to oligo metastatic sites compared to observation. - Upfront radiation administration led to a significant delay in the initiation of ADT. - The trials primarily involved men with previously treated localized prostate cancer showing one to three oligo metastatic lesions. 26:01 *🧬 Italian Multicenter AR Trial Results* - This trial examined castrate-resistant oligo metastatic prostate cancer, with 60% having two to three bone-only lesions. - Patients who received radiation had higher rates of undetectable PSA and improved progression-free survival. - The analysis indicated no reach of median overall survival, suggesting potential longer-term benefits of treatment. 27:22 *📑 ESMO Guidelines for Prostate Cancer* - Differentiation between low and high metastatic burden affects treatment, with ADT as a central component. - Low metastatic burden patients benefit from ADT plus radiation, while high burden patients rely on systemic therapy. - Guidelines suggest involvement of radiation for newly diagnosed oligo metastatic prostate cancer following the Stampede trial insights. 29:09 *👩‍🔬 NRG BR-Z2 Oligo Metastatic Breast Cancer Trial* - Analysis of a trial involving stage four breast cancer patients with up to four metastatic lesions showed no significant survival benefit from radiation or surgery to metastatic sites. - Majority of patients had indolent disease, hormone receptor-positive tumors, and received well-controlled local-regional therapy. - Takeaway suggests low-benefit for oligo metastatic radiation within the first years, with a potential benefit for patients with more than one metastasis over a longer follow-up period. London' s Victoria hospital..LHSC.. in Ontario..has a platform they put on the table that has arm rests under your forearms and under your upper arms so you can completely relax and you don't have to hold on to sticks...and it keeps you in the correct position and it just lifts off the table in case the next patient isn't there for chest radiation
