Im using worsted weight yarn for the book cover, would i still use a 5mm hook? 50 sts on it was very big width wise. I tried a 3.5mm and its about 12in long

12" is probably going to be a bit large for books, especially paperback. You could keep going down in hook sizes, but it will become increasingly difficult to work with, and the fabric will be very tight. The other option if you want to use the worsted and not a DK is you could adjust the stitch counts, but then you'll have to figure out the decreases for the flap as it would likely be less than I use in the video (which could work if you are getting a larger row height as well as larger stitches).
