It's a good chance that this was a Montgomery mod due to the old Innovation fixtures, but I think that this elevator was something MUCH older prior. What kind of door equipment did this have?

I agree, terrible placement of those buttons. I rang the alarm a few times in a Gen2 in the Prudential building by mistake since I'm used to the door close button being there.
