@@DrSujathasENTClasses mam I m from hyderabad here I have shown to ENT doctor ,my ct scan say mild soft tissue noted in meso and hypo tapnum of left ear cavity and soft tissue density along bony external auditory canal. Doctor says if antibiotics don't cure it we have to do a small surgery, mam everytime I m lying down fluid blocks my ear and I feel very uncomfortable. When I m awake I keep pulling my ear to reduce fulness, plz advise me if surgery will improve my blockage and fullness.

see from 6.47. Bridge and ridge are part of posterior canal wall. Bridge overlies aditus ad antrum. Ridge overlies vertical part of facial canal. Ridge is lowered by drilling bone using mastoid drill

1. So mam ridge is , overlying bony part ,lateral aspect of vertical part of facial nerve ,,,,,,I thought it's anterior aspect bony covering of facial nerve. (correct me if iam wrong) 2. Ridge extent is just. From middle ear posterior wall just lateral aspect of facial nerve ,,,or it is purely,, posterior external auditory canal ,,?????????,,,,, please solve my doubt mam

scutum is a sharp bone of superior wall of bony external auditory canal which joints with lateral wall of middle ear. (Above tymapnic membrane in a way) It forms lateral boundary of attic.

I had mastoidectomy and they took occicles I guess it's canal wall down because of cholesteatoma Please could you tell me ... Am I completely deaf now? The choclear is working good no problem with it . I had perfect hearing before surgery and they said I'll probably be deaf .. I don't want an metal implant if they suggest it ...from your experience can they put artificial bone in my ear to connect tympanic menbrene to the choclear ??? I'm so scared that I will not be able to hear if the occicles gone ...and it's affecting my mind ..so I was thinking if they can put artificial bone in there to help hearing ?? I had surgery today Thank you for your time

@@DrSujathasENTClasses thank you for asking I'm OK some pain I took bandage off and it seems they decided to go through the front of my ear instead of back of ear ..wen I woke up all he said was the occicles were eroded and took them that's all I know I have not had biopsy back but I guess they managed to get the cholesteatoma out .. I'm so sad that they took occicles.. I hope that it was not canal wall down with endoscope ..he must have clean the opacification of mastoid aircells .. They never said anything to me after surgery just so not happy ...they may have reconstructed the middle ear with implant I don't know ... I have been trying to research wat they can use I would be happy with Cadaver occicles artificial bone or stem cells therapy .. All I know is they took occicles nothing else so disappointed

I have been reading and watching videos and listened to class and still found this topic confusing...thank you for your straightforward approach ma'am. Drawing the diagrams as you went made it so clear.. really effective and concise teaching, you're a great educator. Ma'am, a question, after all of these surgeries, for example cortical mastoidectomy, after we drill down the bone, is the bone replaced in any way? Does it just heal with fibrous tissue/scarring? I saw in a book that mastoid air cells are coverted to a single large cavity and that confused me...does the outer shell of bone grow back? After this procedure, can you palpate mastoid process on the patient?

Bone that drilled , never grows completely. there will be a cavity ,called postoperated cavity. we are not drilling mastoid process. drilling happens around mastoid antrum ,surface marking of which is Maceven's triangle.Please see my class on xrays,by which you will get an idea of cavity
