when you increased the number of sampling why you didn't add up the probability of success of 2 or 3 , 10% of 40 is 4, so the shipment will be accepted when S=0,1,2 or 3, this will result in a higher prop of acceptance

tough because you didnt really explain the last 4mins properly. In a real case scenario you would not know the probability of defect - what then??

That's true, in a real case scenario you would not know the probability of defect. But the purpose of this example is to demonstrate, mathematically, why increasing the sample size reduces the risk of making a bad decision. While you may not know the true defective rate for the population, you can consider several different defective rates (like a worst case scenario) and make educated, probability predictions based on those assumptions so the risk is as minimal as you are willing to take.
