Doctor Andrea Furlan demonstrates a relaxation technique called Square Breathing or Box Breathing. This breathing strategy is helpful for people who have chronic pain and anxiety. When the person feels that anxiety and fear are interfering with their life, they may take a 2-minute pause and do this relaxation. It activates the parasympathetic nervous system and counter-balances the sympathetic nervous system. The sympathetic nervous system is the stress response of fight or flight. The parasympathetic nervous system is the rest and digest system. During stressful situations, especially related to pain, it is helpful to stop and think, before the anxiety hormones and neurotransmitters of cortisol and noradrenaline are released.
It is important to remember that if the person has a psychological condition of excessive fear or anxiety, they need to seek the help of a doctor, psychiatrist or psychologist. This video does not replace counselling by a medical professional or mental health specialist.

I suffered anxiety for last two years .My Dr told me to take medicine but I didn’t . I started to do this excises it’s helped me lot .we Indian call yoga breathing excises that I new from India . From last 40 years I lived in uk .I am very glad that your all videos are so helpful for people . God bless u . 🙏🌹

My second time hearing this, but this time I needed it. Went through two years of chest pain. Needed new heart meds and nitro patch added. Then I realized some of the pain wasn’t heart related. Turns out I had a slow releasing stomach. Lost lots of weight. Meds for that just started helping when I broke my foot weeding. Anxiety for sure. New pain. Stuck at home for eight weeks. No driving. Just feeling well enough to swimming in the ocean…..not until next year now. so this square breathing will be helpful. Thanks again.❤️

Thank you for your clear explanation. I had forgotten about this technique but I have just been practicing again and I will continue. As I have high blood pressure I don't hold the breath long after inhalation. I do need to build up my strength again

@ p t Hi! As a fellow breather, I'm sending you encouragement! Try for just 2 counts instead if you can't make it to 4. Also, be sure to be filling your lungs all the way from the bottom up. (This sometimes takes practice - you might need to look up a video for correct breathing). Then when its very comfortable, increase to 3 then do the same until you can get to 4. Its like exercising, not everyone can do the same amount because we aren't all in the same condition. I hope you don't mind my words of encouragement. I'm a retired RN plus have Depression with GAD & was taught this technique by another name. I have a whole bunch of other medical diagnoses related to my Disability so I use this technique often. Stay strong!

@ Katie Did Hi Katie! Fellow Chronic Painer here. That's a good start with the Magnesium & Vitamin D3. I've had my CP along with Fibro for 20 years then tossed in Osteo Arthritis for fun about 7 yrs, & am allergic to OTC & most Rx pain relief. With my Healthcare Providers' permission, because of my background as a RN, they let me explore Supplements and Vitamins along with ones they suggested. If I may, I would like to add my list for you to review & look into: Depending on if you had a blood level drawn, women are notoriosly low in Vit D3, so can easily take 5,000 I.U. daily. (I take it 2x daily in Minnesota). My choice is Nature Made™ because of their outside independent testing. Although my Dr. prefers Magnesium maleate the Magnesium Oxide is a close second. I use MagOx™ as directed. By taking the 2nd one closer to bed, it can also help to fall asleep if pain makes this difficult. When using Magnesium Maleate, its best to mix it in water or a juice and sip on it through the day as it's short acting. In addition, soaking in a tub with Epson salts when youre espescially in pain, then getting into a warm bed is a good non- drug treatment. Depending on the source of your pain - bone vs. muscle, Calcium helps more for the bone pain (unless you have kidney or stone issues).I use Citracal™ as directed. Calcium Citrate is highly absorbable where Calcium Carbonate is not. It also has other "assistive Vitamins and Supplements to enhance its absorption rate and effectiveness. For muscle pain, Omega-3 is helpful. I take 1200mg twice a day. Again, the brand is Nature Made™. The important part of Supplement use is being patient while your body finally reaches the correct levels of the different nutrients and begins utilizing them where they are needed most. Then its a matter of tweaking the doses. Right now is the perfect time to go online and find "Pain Diaries". Find one or combinations of some & create your own start filling out now as you're waiting for changes. Then you will be able to tell when the changes start occurring. Trying to say, "Oh, I know what my normal "pain" is like; I'll know when it changes," is the changes may be too subtle for you to pick up on them. Plus, it's *more* than pain; its sleeping, how easily can you get up, how long can you do an activity before pain stops you? How long does it take for that pain to subside? What ADLs or chores can't you do? Does the change in weather affect you? (Download a weather App with Barometric Pressure & check it when your pain gets worse *and* when it suddenly gets less (if you pick up a pattern (sometimes what's going on with the weather outside the window isn't the problem; it's in the Barometric Pressure.) So, look through the different Pain Diaries. I hope something helps here. Stay strong! I hope you have a good Healthcare Provider as your partner in this journey. @ Meera Singh Hi Meera! Another Chronic Painer here (among my many other Medical & Mental Health issues created from my Disability. It sounds like you don't have any medication you can take to relieve your pain? That's so difficult. Can I ask why you aren't on any medication, treatments or complementary medicine?

This is going to be so helpful to me and I thank you so much ! as I am having lately hard time to breathe and even feel like I am suffocating at times and yes, when I am in pain of migraine it is even worse. same happens at night and I am suspicious of sleep apnea How I need to learn how to breathe again. @ Linda Ellis Hi, Linda I watched the video & was scrolling through the comments & saw yours. Having had this in th the past, I have a couple questions: #1.Have you been a *passenger* in a car that was in a rear - end collision, while wearing a seatbelt? #2. Have you had an X-Ray of your Cervical - Thoracic spine. #3. Does your left mid forearm ever feel tight like there is band around it or your wrist feel tight? #4. Have you had Carpal Tunnel surgery on your left wrist but it didn't help? If you answered *yes* to #1, 3 and 4 but *no* to #2, I would like you to take this list to your Doctor & request this X-Ray. As I said, having had this in the past, *these symptoms* are caused from a pinched nerve (I can't recall at what height - low cervical, high thoracic). This nerve also circles to the front of the chest, and when the back muscles spasms in pain, it crushes the vertebrae together & will mimic angina. Most people lay Dow because of the chest pain but it's relieving the pressure on the spine at the same time. And - angina is relieved! After a severe rear end collision (in which I had a undiagnosed TBI for 7 years), I ended up in the emergency room a few weeks later getting an EKG at 30 years old. My P.T. put 2& 2 together when I told him a couple days later & did a spinal release. At least it helped for that. Oh, I'm also a retired RN, if this helps;)

Doctor, Thanks for explaining this technique. Can you please let me know if there is a way to naturally fix trigger finger problem. I am a diabetic and I have a trigger finger problem with the third finger of my right hand. In the mornings, especially, the finger gets folded and stuck for a while. I would have to massage a bit at the corner of the palm and that finger and then use my left hand to unfold/unlock the locked finger of my right hand.

Good madam square box breathing 5 cycles reduces fear & anxiety prevent chronicity of pain .as you suggest for chronic lbp i used to advice deep diaphraghmatic breathing techniques in supine position clubbed with our routine back core exs ..it reduces sensitivity gives good relaxation and positive confidence ..drastically reduces phobia ..its very useful info to all. My suggestions already you had demo video on knee pain why dont you specific on Oa knee good exercises rather than traditional quads. For example if a person not able to squat then our training focused on deep knee flexion activity instead quads exs ...? Am i right so meaningful physio ..please publish with evidence based info.my simple suggestions.

Kwadrat jest zdecydowanie lepszy bo liczenie do 5 jak proponował mi fizjoterapeuta to już wydłużenie oddechu i przez ćwiczenie do mistrzostwa, ale do 4 to ma znaczenie? Diaphragmatic breathing is great! It is the technique that singers learn to breath while they are singing. And Singing is a great way to activate the parasympathetic nervous system.

Add the anxiety of trying to “age in place.” I don’t want to live in a flat clean room with no pain or worry. Sounds like pre coffin life. I want to accept the reality of my life. That means I am not expecting anything to be different. It just is.

@ Rose K Hi Riose, I watched the video too, but realized I already did this technique. I just knew it by a different name. I use it at night if I get insomnia. I use while I am already in bed, in my usual sleep position. Then I start. I would suggest staying with only the four count, however. The shorter time will be best for sleep. What you might try if still having trouble, is come up with a mantra. (I have one I was already using for my pain and incorporated it. I had to add a couple extra lines for the breath holds. IN: "In with Peace and Healing" HOLD: "Let them fill all of my body" OUT: "Out with Stress and Pain" HOLD: "Let them disintegrate like dust." Couple tips for sleep: >Turn off Electronics at least 30 minutes before bed so your brain can "power down". >Make sure all the little lights on appliances are covered & light from outside is blocked or wear a sleep mask. Having ambient light keeps your brain "awake". Your brain needs to enter REM sleep which is the Restorative sleep stage. >Keep the temperature in the Bedroom cooler than warmer. You're body relaxes better when its cooler. 72° F or lower is good. >If noise is bothersome use an App with ambient noise, play music without words or run a fan on low to mask sounds. Or, earplugs. Hopefully, something helps here for you.

Hi VK, I believe that education is part of the intervention. If people do not know and undersand why they are doing the exercises, they will not adhere to the routine. I believe that people like to learn, and just for your information, my most viewed videos (more than 1 million) views are the longer videos and those that I spend time teaching the concepts, not just telling them what to do. If you dont' like my videos, you may find other channels that do not explain anything.
