There are some other versions of the first phrase "Are you kidding me?" that I didn't mention here. We can actually replace 'kidding me' with a number of other options:
Are you joking?
Are you having a laugh?
Are you taking the Micheal?
Are you taking the Mick?
Are you taking the Micky?

With "how dare you", if it's a little more playful, I might use "how VERY dare you" accompanied with a posh accent. It's like saying "you cheeky little...", for example when someone makes a joke that's a little personal or insulting, but in good humour.

For the UK definitely. For the US, some of them I’ve heard in American movies but you would have to check with an American for how common they are in day to day use.
