The foot stuck in the cage thing is rare, but it does happen. I watched an amateur defend a takedown only to have his knee popped by it. His opponent actually backed off of him and called the ref and coaches over to help him. The ruling was a TKO and the crowd lost its mind they were so pissed.

As an asthmatic I have to say that the use of Corticosteroids mid fight is insane, back when I was younger I used a spray inhaler with those, instantly after use the pain numbs and the lungs open while you get a considerable surge of strength

I feel bad for the guy who got his nuts obliterated. Honestly if it was still that painful after 5 minutes, some damage was probably done. It would be almost impossible to take that “loss” gracefully.

And these are examples why individual athletic commissions are a bad thing. I don't think the argument for those knees to the downed opponent were delivered accidentally. I think they meant the attacker didn't realize his opponent was down when those knees were thrown.
