The alternating star wheel mechanism makes me think of the cam lobe design that Charlie Taylor used for the engine that he built for the Wright brother's machine shop in Ohio.

Excellent - How on earth does the governor work? I can see the latch under the exhaust valve, but I can't for the life of me see what activates it! (Edit - is it that weight swinging backwards and forwards on the eccentric shaft?? )

It is a pendulum governor. If the engine is going fast enough the momentum swings the weight up to latch open the exhaust valve. If going too slow it doesn’t swing enough and allows the engine to cycle again.

I think it is very fuel efficient. It fires then coasts for several revolutions before firing again. I have run it for five hours and it used half a gallon of gasoline. The low RPM’s reduce heat and wear. As modern engines got smaller they had to increase RPM to obtain the same horse power.
