The ngative impacts of headphones are not due to how the sound travels through your ear. The SPL of those waves will be the same... 60SPL with speakers is the SAME as 60SPL with headphones. The wax buildup and infections are because you have little plastic things with microbes on them inside your ear. If you just wore in ear headphones and never listened to them you would get the same effects.
