I've never seen a good mold that did not have an exhaust hole. You know it's full when excess starts to exit the exhaust hole. Graphite is easy to carve on so you can modify them.

It looks to me like the melt was freezing in the mouth of the mold. Perhaps, it would help if you made that spot particularly hot.

I agree, I'm guessing the mold needs to be hotter. I bet if you position a propane torch blowing on the side of the mold while you pour that this will let you achieve better results.
